Change reasons for assessment (RFA)
Path: Patient>Clinical>Assessments
You can change an RFA only to another RFA within the same subset because of the vast differences between the templates in each subset. For example, Start or Resumption of Care assessment RFA can be changed only to another Start or Resumption of Care assessment RFA.
When you change an RFA, an assessment is reinitialized based on the new RFA. Tabs are changed based on the new RFA. Answers you already entered remain intact unless there is no corresponding item in the new template, and in this case, they are deleted.
When you validate the completed assessment, any warning triggered by the new template appears.
In the Patient component, select a patient.
- Go to Clinical>Assessments.
- If you have the privileges to change the documentation of other staff, a dialog box appears stating that although this is a completed assessment, you have the privileges to change it. Click OK.
- In the Confirm dialog, click OK to reserve the assessment.
- Complete the Reserve Assessment dialog, and click OK.
- Select the assessment whose RFA you want to change and click Change RFA.
- Select the appropriate RFA.
- Save the assessment.
Note: Starting from 15.2 assessment templates version, you cannot change reason for assessment (RFA) for non-Oasis and non-HIS tabs.
