Run Acute and Emergent Care Risk assessment

Path: Patient>Clinical>Assessments

Using the Acute and Emergent Care Risk Assessment functionality, you can identify a patient's acute and emergent risk level based on the OASIS export record. The risk level can be displayed as a grid diagram (general information), as a window (detailed information on risk factors), and as a report (for printing).

The Acute and Emergent Care Risk Assessment is run when the following requirements are met:

  • Predictive Model license is selected for the Business Unit.
  • Selected assessment is OASIS assessment with RFA1 (Reason for Assessment), Start of care.
  • Assessment is 100% complete.

To run the Acute and Emergent Care Risk:

  1. In the Patient component, select a patient.
  2. Go to Clinical>Assessments, and select the appropriate OASIS assessment.
  3. Click the Predictive Probability of Risk button ( ).
  4. The Predictive Probability of Risk dialog shows the Acute Care Hospitalization and Emergent Care patient risk factors based on OASIS export record.
  5. The risk level color is shown in the appropriate position on the grid: dark green (Very Low Risk), light green (Low Risk), yellow (Moderate Risk), orange (High Risk), red (Very High Risk).
  6. Only one risk level is shown while the other cells are blank. The explanation of the patient's risk level is shown on the word balloon of the same color as the risk level in the grid.
  7. Click the corresponding risk level in the grid to see the Risk Factor Detail window.
  8. Click Printer Friendly Version to get the predictive probability of risk report. The report contains the following information:
  • Patient Name – Name of the patient for whom the Risk Factor detail is shown.
  • Patient Number – Patient code.
  • Assessment Type – Type of the assessment.
  • Assessment Date – Date when the assessment was provided.
  • Completed By – Name of the resource who provided the assessment.
  • Patient's risk level (Acute Hospitalization or Emergent Care).
  • Risk factors grouped by levels.

You can print, save, change the view mode, and navigate through the report using the standard report window buttons on the top panel of the window.
