Assessments window - Revision History tab

Path: Patient>Clinical>Assessments

The clinician's electronic signature is applied when an assessment is completed, saved, and uploaded from Field to Host Mode.

Signed clinical documentation cannot be altered without corresponding privileges.

The signature of the assessment from Field Mode is logged in Audit Trail. If an assessment is created and saved in Host Mode, the electronic signature is applied (shown in the footer of the assessment). However, the signature is not captured through the Audit Trail.

For assessments, the complete audit trail is maintained:

• Original entry
• Date and time when the change was made
• The change itself
• Who made it

This information is shown on the Revision History tab. Revisions to assessments require that the assessment is reserved. For details about assessments revision history, see About assessment revision history.

You can copy the revision history content by right-clicking anywhere in the Revision History window.

Note: This tab provides the revision history for assessments only. To view the revision history of changes made to the patient's basic and clinical information, see Revision History Log Panel.
