OASIS Assessments: Templates
The OASIS questions are integrated into these assessments:
- SN (Skilled Nursing)
- PT (Physical Therapy) with treatments
- PT (Physical Therapy) without treatments
- OT (Occupational Therapy)
- SLP (Speech Language Pathology)
- Psych (Psychiatric Nursing)
There is also an OASIS Only assessment for those agencies that do not use field devices.
The appropriate OASIS questions appear in the assessment based on the Reason for Assessment (RFA) selected by the clinician in the New Assessment window.
Note: As of January 1, 2020, you need to use OASIS D1 templates. If you attempt to use an OASIS D1 template prior to January 1, 2020, you will see a warning message. If you attempt to use an OASIS D template after January 1, 2020, you will also see a warning message.
The only exception to this would be a recertification assessment being entered on December 27, 2019 - December 31, 2019 for recertification dates of January 1, 2020 or later. The MO090 date for OASIS D1 assessments that are used between December 27, 2019 and December 31, 2019 will automatically update to January 1, 2020 and cannot be exported before January 1, 2020.
