Deactivate HIS forms exported

Path: Patient > Documents > Hospice Item Set

Prerequisite: To deactivate HIS forms, you must be granted appropriate privileges by the Netsmart Homecare administrator.

If you no longer need a HIS form, or if errors were found in a signed HIS form and you need a new one, you can deactivate it.

  1. Select a patient, and go to Patient > Documents > Hospice Item Set.
  2. Select the HIS form that you want to deactivate, then click Inactivate. The status of the form is Exported. Note: Exported forms that were modified cannot be deactivated.
  3. After you deactivate the form, the following occurs:
    1. An inactivation request is sent to the QIES ASAP system.
    2. The deactivated form is saved for historical purposes with 3. Inactivate existing record selected in A0050. To view this form, select the Show Inactive Forms check box. On the Preview tab, you can save it in the PDF format.
    3. A new HIS form is created as a duplicate of the deactivated form and shows the signature of the person who originally verified the document (in Z0500A) and the date and time of that signature (in Z0500B).
  4. Edit the Patient and Form Identifying HIS Items in the new HIS form, but do not edit items that contain the patient's clinical record. Important: Ensure that responses in this form are also reflected in the patient record.
  5. Click Validate to ensure that the HIS form meets the QIES ASAP system requirements, and correct the errors, if any.
  6. Click OK, and then electronically sign the form by entering your operator's login password in the Electronic Signature window. 
    Your electronic signature is recorded in Z0400 after you save the form.
  7. Review the HIS form to ensure that all HIS items are complete and that an authorized user can verify the form by Provide final signature to HIS forms.
