Create and prefill HIS Discharge form

To create and prefill a HIS discharge form, complete the following steps:

  1. Ensure that the HIS admission form exists.
  2. Follow the Rules for creating HIS forms.
  3. Complete the following assessment for a patient's discharge using the versions that contain HIS items:
    1. SN Hospice - Define the discharge/transfer summary, select a type of visit other than Comprehensive, and then select Discharge/Transfer on the Symptom Management tab.
    2. IPU - Select Routine visit or Oncall/unscheduled visit as the reason for the assessment, and define the discharge/transfer summary. 
    3. Note: If you complete an assessment and then define a patient as discharged in Patient > General > Admissions & Status, the HIS discharge form is created with a blank A0270 item. To create a completed HIS discharge form, first discharge a patient, and then complete a patient's assessment.
  4. To save the completed assessment containing the HIS items, enter your operator's login password in the Electronic Signature window. Your electronic signature is recorded in Z0400 of the generated HIS form. Note: After you electronically sign the form, it cannot be deleted.
  5. If needed, you can edit the HIS form created from the assessment. 

Important: All changes to the HIS items in the assessment are also reflected in the HIS form. Manual edits in HIS forms, however, are not reflected in the assessment.
