Complete HIS forms manually
Path: Patient > Documents > Hospice Item Set
Prerequisite: To complete a HIS form manually, the Allow Manual Entry check box must be selected in Administration > Configuration > Business Units > Settings > Administration.
If you do not use assessments that contain the HIS items and work in host mode, complete the following steps:
- From Patient > Documents > Hospice Item Set, add a new row, and then select the needed form type in the Select Form Type window.
- In the HIS Form Details window, complete the following sections:
- Section A: Administrative Information. For most items in this section, Prefilling of General Patient Data in HIS Forms occurs. If needed, you can make edits in the prefilled HIS items; however, ensure that the data in the HIS form is consistent with the data entered in other places in the application. The A0050 item is not editable and is completed automatically based on the HIS form status.
- Section F: Preferences.
- Section I: Active Diagnoses.
- Section J: Health Conditions.
- Section N: Medications.
- For item-specific instructions, see the HIS Manual at www.cms.gov.
- Click Validate to ensure that the HIS form meets the QIES ASAP system requirements, and correct any errors.
- After you document the needed HIS items, click OK, and then electronically sign the documented HIS items by entering your operator's login password in the Electronic Signature window. Your electronic signature is recorded in Z0400 after you save the form.
- Important: After you electronically sign the form, it cannot be deleted.
- Preview and, if necessary, edit the HIS form by clicking the ellipsis button in the Details column or double-clicking the HIS form record. Note: Ensure that all HIS items are complete and the person who is verifying the form can proceed with Provide final signature to HIS forms.
- Save your changes.
