Create and prefill HIS Admission form

Path: Patient > Documents > Hospice Item Set

1. Follow the Rules for creating HIS forms.
2. Define a patient as admitted or readmitted in Patient > General > Admissions & Status.
3. Complete one of the following assessments for a patient's admission using the versions that contain the HIS items:
   1. SN Hospice - Select the Initial Visit as a reason for the assessment and Comprehensive type of a visit.
   2. IPU - Select the Initial Visit as a reason for the assessment.
4. To save the completed assessment with HIS items, enter your operator's login password in the Electronic Signature window.
   1. If these assessments do not contain information related to the Hospitalization Preference F2200/NQF 1641 and NQF 1647, then these items can be provided within the Social Work and Spiritual assessments, and they will be prefilled on the HIS admission form.
   2. Your electronic signature is recorded in the Z0400 on the generated HIS form. Note: After you electronically sign the form, it cannot be deleted.
5. If needed, edit the HIS form created from the assessment.

Important: All changes to HIS items in the assessment are also reflected in the HIS form. Manual edits in HIS forms, however, are not reflected in the assessment.
