Record pre-admission information - PM

The Pre Admit form allows you to record the demographic, basic physical, and mental health history for a client who has not been admitted into an admission program.

  • This form can be used for a facility waiting list, or when a client is given a billable assessment before it has been determined which treatment program the client will be admitted to.
  • Clients entered in this form can be scheduled for services and have services billed for them.
  • By default, admissions are not permitted to be processed from the BUILD environment. To enable this functionality, please contact your Netsmart representative.
  • Clients must not have an active admission episode (no discharge date).

Note: Only one pre-admission episode may be open at one time. This behavior can be modified via a Registry Setting (Allow Active Pre-Admits And Admits must be set to 'M').


  1. Go to: Avatar PM > Client Management > Episode Management > Pre Admit
  2. In the Select Client screen, enter the client's last name, first name, and sex. Select Search.
    • If the client is already registered, they will be found in the search results.
      • Select the client.
      • If the client has previous admissions, the Episode Information pre-display shows. Choose an existing episode, or select Add.
    • If the client is not found, select New Client. The Auto Assign dialog displays.
      • Choose Yes to generate a medical record number.
      • Choose No to display the ID Number dialog and enter an ID number.
  3. The Facility field displays the facility number.
  4. The Episode Number field displays the client's episode number.
  5. The Client Name and Sex fields display the client's name and gender, according to existing information, or your search terms in the Select Client screen.
  6. In the Date Of Birth field, enter the client's birth date.
  7. The Age field displays the client’s age based on the Date Of Birth.
  8. In the Pre-admit/Admission Date field, enter the pre-admission date. If appropriate, the date can be changed in the Change Program/Admission Date form.
  9. In the Pre-admit/Admission Time field, enter the pre-admission time.
  10. In the Program field, select the program. Programs are defined as pre-admission in the Program Maintenance form (Program Type field).
  11. The Treatment SettingTreatment Service, and RRG fields display treatment information for the episode, if available.
  12. In the Type Of Admission field, select the admission type.
  13. For the Admitting Practitioner field, enter the practitioner name or ID, and select.
  14. In the Expected Date Of Admission field, enter the date.
  15. In the Scheduled Admitting Practitioner field, enter the practitioner name or ID, and select.
  16. In the Social Security Number field, enter the client’s social security number.
    Note: Access to the Social Security Number field is defined in the User Definition, and User Role Definition forms (Social Security Number Access field). This is defined by a system administrator. The user may be able to:
    • Enter new data for a client, edit and view data.
    • Enter new data for a client, and view data.
    • Enter new data for a client.
  17. In the Pre-Admission Diagnosis field, enter the diagnosis name or code, and select.
    • Axis I diagnosis codes are available.
    • Diagnosis codes are set up in the RADplus Utilities Diagnosis Table Maintenance form. 
    • Note: The pre-admission diagnosis is not the default billing diagnosis.
  18. In the Ever A Client At This Facility field, select Yes if the client has previously been in the facility.
  19. In the Employer field, enter the client’s employer.
  20. In the Disposition field, select the pre-admission arrangement made for the client.
  21. In the Pre-Admission Disposition Details field, enter comments associated with the pre-admission.
  22. The Team Assignment field defaults to a team based on the Program selection (this association is set up in the Team Definition form). The client will be added to the caseloads of the users assigned to the selected team.
  23. When finished, Submit.
