Enter admission information - outpatient - PM

Admit clients into an outpatient treatment program.

  • The Admission form is used to admit clients into inpatient and partial hospitalization treatment programs.
  • The Netsmart InfoButton can be added to the bottom of the main Admission section of this form to allow you quick access to the Medline Plus webpage. Please contact your Netsmart representative to have this enabled in your system.
  • Clients who are admitted through the Pre Admit form must be discharged in the Pre Admit Discharge form.

Prerequisites:


  1. Go to: Avatar PM > Client Management > Episode Management > Admission (Outpatient)
  2. The Client Name field displays the client name.
  3. If appropriate, in the Sex and Date Of Birth fields, enter the appropriate information.
  4. The Age field displays the client age.
  5. In the Facility Chart Number field, enter the number.
  6. In the Social Security Number field, enter the client’s social security number. Access to the Social Security Number field is defined in the User Definition, and User Role Definition forms (Social Security Number Access field) This is defined by a system administrator. The user may be able to:
    • Enter new data for a client, edit and view data.
    • Enter new data for a client, and view data.
    • Enter new data for a client.
  7. In the Pre-admit/Admission Date field, enter the admission date. If appropriate, the date can be changed (Change Program/Admission Date form).
  8. In the Pre-admit/Admission Time field, enter the admission time.
  9. In the Program field, select the outpatient treatment program.
    Note: The Allow Admission To File/Edit Pre-Admits registry setting can affect the Program field.
  10. The Treatment Setting, Treatment Service, and RRG fields display information associated with the program.
  11. For the Type Of Admission field, select the admission type.
  12. In the Source Of Admission field, select the admission source.
  13. In the Admitting Practitioner field, enter the practitioner name or ID, and select.
  14. In the Attending Practitioner field, enter the practitioner name or ID, and select. Attending practitioners can be re-assigned to the client throughout the client episode.
  15. In the Practitioner Type field, select the type of practitioner.
  16. In the Disposition field, select the client's mood.
  17. For the Reason For Contact field, select the reason.
  18. In the Custody field, select the client's custody status.
  19. In the Custody Effective Date field, enter the date, if applicable.
  20. In the Perform Discharge Alert field, select Yes to display a warning message when a client is discharged or goes on leave. Discharge alerts are managed in the Discharge Alert Input form.
    • If Yes is selected, in the Type Of Alert field, select the alert type.
  21. In the Permission To Contact After Discharge field, select Yes if the client can be contacted after discharge.
  22. In the 'Presenting Problems' fields, select the client problems present when the client was admitted.
  23. In the Accompanying Person Relationship field, select the person's relationship to the client.
  24. In the Accompanying Person Name and Address fields, enter the name and address of the person accompanying the client (if available).
  25. In the Received Copy Of Client Rights field, select Yes if the client received a copy of their rights.
  26. In the Advanced Directive field, select Yes if the client submitted an advanced directive.
  27. In the Advanced Directive Note field, enter advanced directive information.
  28. For the Allow Visitations field, select Yes if the client is allowed to receive visitors.
  29. In the Automatic Room And Board Advanced Days field, enter the number of days.
  30. If applicable, enter the appropriate policy number in the Policy Number for 270/271 Real Time Processing field. This entry is be used to populate the 2100C NM109 segment in the 270 submission file.
    Note: This field is controlled via the Enable Real-Time 270/271 Submission registry setting, when 270/271 submission is enabled.
  31. If applicable, select Child or Individual in the Coverage Level For 270/271 Real Time Processing field. This entry enables the user to toggle 'Child' or 'Individual' in the 2110C EQ03 segment in the 270 submission file.
    Note: This field is controlled via the Enable Real-Time 270/271 Submission registry setting.
  32. In the Team Assignment field, assign the client to a team practitioner's caseload. 
    Note: The client cannot be assigned to the team caseload if the team assigned during admission has the Disable Adding Clients to Caseload field set to Yes on the Team Definition form.
  33. In the Is this a Transition in Care? field, select whether or not this admission marks a transition in the client's care. This field is used to record incoming transition of care that typically come from a referral for meaningful use measures.
    • If Yes, enter the dates for the prior admission in the Previous Hospital Admission Date, and Previous Hospital Discharge Date fields.
  34. When finished, click Submit.
