Source Of Admission - Dictionary Value

Source Of Admission is set up in the Dictionary Update form (Client file - Data Element 158).

 

It is recommended to use Industry Standard Billing values for this dictionary:

  • Code = 1; Value = Non-Health Care Facility Point of Origin
  • Code = 2; Value = Clinic or Physician's Office
  • Code = 4; Value = Transfer from Hospital (other facility)
  • Code = 5; Value = Transfer from SNF, ICF, ALF or NR
  • Code = 6; Value = Transfer from other Health Care Facility
  • Code = 8; Value = Court/Law Enforcement
  • Code = 9; Value =  Information Not Available
  • Code = D; Value = Transfer from One Unit to Another Unit
  • Code = E; Value = Transfer from Ambulatory Surgery Center
  • Code = F; Value = Transfer from Hospice Facility

These recommended values may change over time based on industry changes.

 

This dictionary has extended dictionary elements:

  • Client File - Data Element 1399 - Source Of Admission (2300-CL1-02/UB92-FL 20/UB04-FL 15)
    • Select the Source of Admission that will populate the 2300-CL1-01 of the 837 Institutional billing file and box 14 for UB04. This Extended Data Element is required to be mapped for each Source of Admission dictionary value.

  • State Forms File - Data Element 14005 - State Reporting Referral Source
  • State Forms File - Data Element 61001 - Florida - Extended Value For AHCA Reporting
  • State Forms File - Data Element 55013 - VTSR MSR Source of Referral
  • State Forms Table File - Data Element 55021 - VTSR SATIS Principal Source of Referral

  • Pre Admit
    • This is enabled via the Enable Additional Fields On Pre-Admit Option Registry Setting
  • Admission
  • Admission (Outpatient)
  • Back Date Admission/Discharge

 

Applicable Registry Settings:

  • Require Source of Admission of Admission for Outpatient Admissions
  • Enable Additional Fields On Pre-Admit Option

 

Additional Information:  

  • The maximum dictionary code length is two (2) characters
  • The maximum dictionary value length is forty (40) characters
