Pre Admit Discharge - PM

Discharge a pre-admission client from a pre-admission program.

Prerequisite:

  • A client must have been pre-admitted through the Pre Admit form.

  1. Go to: Avatar PM > Client Management > Episode Management > Pre Admit Discharge.
  2. In the Select Client screen, enter the client name or ID, and select. Click Select.
  3. The Episode Number field shows the client's episode number.
  4. In the Date Of Discharge field, enter the discharge date.
  5. In the Discharge Time field, enter the discharge time.
  6. The Discharge Day Of Week field shows the discharge day.
  7. The Length Of Stay field shows the episode length in days.
  8. In the Type Of Discharge field, select the discharge type.
  9. In the Discharge Practitioner field, enter the practitioner name or ID, and select.
  10. In the Accompanied By field, enter the person accompanying the client.
  11. In the 'Discharge Presenting Problems' fields, select the client problems present when the client was discharged.
    These fields are set up in the Dictionary Update form:
    • Discharge Presenting Problems-Primary
    • Discharge Presenting Problems-Secondary
    • Discharge Presenting Problems-Tertiary
  12. In the Current Resident Code field, select the client residence.
  13. In the Homeless Indicator field, select client homeless status.
  14. In the Discharge Remarks/Comments field, enter discharge comments or observations.
  15. When finished, click Submit.
