Change Program/Admission Date - PM

Use this form to change a client’s date of admission to a treatment program.

The new admission date:

  • Must be before any client movements (leaves, program transfers, or discharge) that occurred in the client episode.
  • Must be before posted services for the client.
  1. Go to: Avatar PM > System Maintenance > Client Maintenance > Change Program/Admission Date.
  2. In the Client ID field, enter the client name/ID, select the Search button, then select OK.
  3. In the Episode Number field, select the episode.
  4. In the New Admission Date field, enter the new admission date.
  5. In the New Admission Time field, enter the new admission time.
  6. If there is a change in program, select the correct program from the New Program drop-down. This options available in this field are dependent upon the client's current program type. Programs can only be changed to another program of the same type (Pre-Admit, Inpatient, etc.).
  7. Select Submit.

Note: RADplus_entity_access.option_description = Change Program/Admission Date and RADplus_entity_access.ENTITY_ID = PATID can be used to see the date/time/user that submitted this form by using the other fields in the RADplus_entity_access table.
