Admission - Inpatient - Cal-PM

Enter clients into an inpatient treatment program.

  • By default, admissions are not permitted to be processed from the BUILD environment. To enable this functionality, please contact your Netsmart representative.
  • The Admission (Outpatient) form is used to admit clients into an outpatient treatment program.
  • If the client is being admitted into a treatment program that is associated with CSI data, a dialog displays when this form is submitted.
  • The client must be admitted into a CSI treatment program in the CSI Admission form.
  • The Netsmart InfoButton can be added to the bottom of the main Admission section of this form to allow you quick access to the Medline Plus webpage. Please contact your Netsmart representative to have this enabled in your system.
  • Clients who are admitted through the Pre Admit form must be discharged in the Pre Admit Discharge form.

Prerequisite:


  1. Go to: Avatar PM > Client Management > Episode Management > Admission
  2. The FacilityEpisode Number, and Client Name fields display the client information already entered.
  3. If you know the Sex and Date Of Birth for the client, enter the information.
    Result: If you add the Date of Birth, the Age field displays the client age.
  4. In the Pre-admit/Admission Date field, enter the admission date. If appropriate, the date can be changed (Change Program/Admission Date form).
  5. In the Pre-admit/Admission Time field, enter the admission time.
  6. In the Program field, select the treatment program.
    • The Allow Admission To File/Edit Pre-Admits registry setting affects the Program field.
  7. The Treatment Setting, Treatment Service, and RRG fields display information associated with the program.
  8. In the Type Of Admission field, select the admission type.
  9. In the Source Of Admission field, select the admission source.
  10. In the Admitting Practitioner field, search for and select the admitting practitioner by name or ID.
  11. In the Attending Practitioner field, search for and select the attending practitioner by name or ID. Attending practitioners can be re-assigned to the client throughout the duration of the client episode. This field is disabled when a client episode is opened for editing.
  12. In the Team Assignment field, assign the client to a team practitioner's caseload. This field is populated based on the Team Definition form.
  13. In the Practitioner Type field, select the type of practitioner.
  14. In the Facility Chart Number field, enter the number.
  15. In the Social Security Number field, enter the client’s social security number.
  16. Access to the Social Security Number field is defined in the User Definition, and User Role Definition forms (Social Security Number Access field). This is defined by a system administrator. The user may be able to:
    • Enter new data for a client, edit and view data.
    • Enter new data for a client, and view data.
    • Enter new data for a client.
  17. In the Perform Discharge Alert field, select Yes to display a warning message when a client is discharged, or goes on leave. Discharge alerts are managed in the Discharge Alert Input form.
  18. In the Type Of Alert field, select the type of discharge alert.
  19. In the Disposition field, select the client's mood.
  20. In the Presenting Problems fields, select the client problems present when the client was admitted. The following fields are set up in the Dictionary Update form:
    • Presenting Problems-Primary
    • Presenting Problems-Secondary
    • Presenting Problems-Tertiary
  21. In the Client’s Living Arrangements field, select the client's living situation.
  22. In the Disabilities fields, select the client disabilities as appropriate.
  23. In the Current Medications fields, select client medications as appropriate.
  24. In the Received Copy Of Client Rights field, select Yes if the client rights was received.
  25. In the Advanced Directive field, select Yes if the client submitted an advanced directive.
  26. In the Advanced Directive Note field, enter advanced directive information.
  27. In the Admission Note field, enter information associated with the admission.
  28. In the Admission Department Time Out field, enter the time.
  29. In the Is this a Transition in Care? field, select whether or not this admission marks a transition in the client's care. This field is used for Meaningful Use measures reporting to mark the admission record (typically coming from a referral) as a transition of care into the facility.
    • If Yes, enter the dates for the previous hospital stay in the Previous Hospital Admission and Discharge Date fields.
  30. When finished, click Submit.
