Enter information for the client that is specific to the agency.

  1. In the Type of Translator Needed field, specify the language of the translator the client requires.
  2. In the Authorized for Ongoing Treatment field, designate whether the client is authorized for ongoing treatment.
  3. In the Additional Comments field, enter any additional comments.
  4. Click the Print Consent for Treatment Form to prepare a consent form for the client to sign.
