Pre Admit - Cal-PM
The Pre Admit form allows you to record the demographic, basic physical, and mental health history for a client who has not been admitted into an admission program.
- This form can be used for a facility waiting list, or when a client is given a billable assessment, before it has been determined which treatment program the client will be admitted to.
