CSI Admission - Cal-PM

Enter required information for a client who has been admitted to a Client Services Information System (CSI) treatment program.

  • CSI data is reported to the California Department of Mental Health.
  • A treatment program must be associated with CSI data collection in the Program Maintenance form.
  • A client must have been admitted to a CSI program in the Admission or Admission (Outpatient) form.

  1. Go to: Avatar PM > Client Management > Client Information > CSI Admission
  2. In the Select Client screen, search for and select the client by name or ID.
    • If the client has multiple episodes, select an episode from the Episode Selection and click OK.
    • If the client contains CSI records from a different episode, the CSI Data Collection dialog displays. Click Yes to default information from another episode.
  3. The Default CSI Information dialog displays. Select the episode, click OK. CSI program information populates and can be edited.
  4. In the 'Birth Name' fields, enter the client's information.
  5. In the Mother’s First Name field, enter the mother's information.
  6. In the Fiscally Responsible County For Client field, select the county.
  7. In the 'Place Of Birth' fields, select the client’s birthplace information.
  8. In the CSI Ethnicity field, select the client's ethnicity.
  9. In the Special Population field, select the client's special population information.
  10. In the Legal Class field, select the client's legal class.
  11. In the County School field, select the county school.
  12. In the District County Code and District/Site Code fields, select the county and site.
    • The District County Code and the District/Site Code fields override the District Of Residence (CSI) extended dictionary in the County School field.
    • The 'S-39.0 District Of Residence' data element will populate with the two character code from the District County Code field, followed by the five character code from the District/Site Code field.
    • The 'S-39.0 District Of Residence' is the California Department of Education issued COUNTY-DISTRICT-SCHOOL (CDC) code provided to local education agencies (LEADs).
  13. In the Admission Necessity Code field, select the admission necessity.
  14. In the 'Affecting Mental Health' fields, select how the client conditions affect their mental health.
  15. In the Conservatorship/Court Status field, select the client status.
  16. In the Preferred Language field, select the client's language.
  17. In the Race (Select Up To Five) field, select the client's race.
  18. In the Number of children less than 18 years of age field, enter the number of children.
  19. In the Number of dependent adults 18 years of age and above field, enter the number of dependents.
  20. Click Submit.
