Demographics Section - Cal-PM

Enter client demographic information.

This section is available on the following forms:

  • Admission
  • Admission (Outpatient)
  • Discharge
  • Discharge (Outpatient)
  • Pre Admit
  • Pre Admit Discharge

  1. Enter the client's Address.
    Note: The County value is from the Client's Address - County dictionary.
  2. Select the client's Communication Preference for contacting them, if applicable. 
  3. Select the client's Primary Language.
  4. Select the Client Race.
  5. If additional races apply, select which ones in the Other Race(s) field.
  6. Select the client's Ethnic Origin.
  7. For Client Declined To Provide Information On The Following, select the categories which the client chose not to disclose.
  8. Select the client's professed Religion.
  9. For Place Of Birth, enter the location where the client was born.
  10. Select the client's Country of Origin.
  11. Enter the client's Preferred Name, if applicable.
  12. Select the client's Marital Status.
  13. For Maiden Name, enter the client's unmarried surname, if applicable.
  14. Select appropriate values for the client's:
    • Education
    • Employment Status
    • Occupation
    • Gender Identity
    • Sexual Orientation
  15. Select the client's Smoker status.
  16. For the Smoking Status Assessment Date, enter the date the smoking status was taken.
  17. In the Alias fields, enter client alias information as appropriate.
  18. Enter the Mother's Maiden Name for identification purposes.
  19. For the Protection Indicator, select whether the client's information may be shared with others. If Yes, enter the Protection Indicator Effective Date for when the protection should begin.
    Note: The Protection Indicator information is for immunization reporting and populates the publicity code and publicity code effective date, respectively, in the HL7 message (HL7 segment PD1, fields 11 and 18).
  20. For the Name Qualifier, select whether there are restrictions on disclosing the client's name.
    Note: This field is for syndromic reporting and is used to determine if the client's name should be included in the HL7 message (HL7 segment PID field 5).
  21. Select the client's Pregnancy Status.
    • This dictionary field accepts user-defined codes. 
    • Pregnancy status is used for reporting purposes to communicate information to RxConnect and does not have an impact on billing.
  22. If the client is pregnant, enter the Expected Due Date given by their OB-GYN. 
  23. For Lactating Status, select Yes if the client is actively breast feeding.
  24. For Veteran, select the client’s veteran status.
  25. Select Submit.
