Clinical Reconciliation Version 1.0

The Clinical Reconciliation form is introduced into myAvatar with the development for Meaningful Use 2014 Edition. The form serves many purposes, including support for Meaningful Use measures reporting. The most important functionality is transforming imported data from a CCD into the Care Record.

The Clinical Reconciliation form is a client based, non-episodic form. Data is imported through a CCD and stored in the CCD SQL table, and will be available for reconciliation when launching this form for the specified client. Data that already exists within myAvatar or is received from OrderConnect will also be available for reconciliation.

If the client does not have an active episode, when the Clinical Reconciliation form is opened, a dialog will display, asking if the user wants to continue to the form. Clicking Continue will open the form, clicking Cancel will return the user to the Home View.

Avatar CWS > Other Chart Entry > Clinical Reconciliation

The Clinical Reconciliation form supports the following functionality:

  • Reconcile Problems imported from a CCD and entered into CWS Problem List.
  • Reconcile Allergies imported from a CCD and entered into CWS Allergies and Hypersensitivities and OrderConnect.
  • Capture Patient Reported Medications in the Home Medication List.
  • Reconcile Medications imported from a CCD and entered into the following locations:
    • OrderConnect Prescriptions
    • OrderConnect Non-ISC Reported Items
    • Avatar Home Medication
    • Avatar OE Medication Orders
  • Run History Reports on Inactive Problems, Medications, Allergies and Home Medications.
  • Run a Current Home Medication List report.

Prerequisites:

The Enable Clinical Reconciliation registry setting must be set to 'Y'.​

  • Avatar CWS 2014 - latest updates

  • Install the latest OrderConnect updates
    The facility must be using the Micromedex catalog

Facilities using myAvatar Order Entry 2011

  • Avatar OE update for RxNorm for medications - OE 2011 Enhancements #44 (U-71), #45 (U-74) and #49 (U-81).

Facilities using myAvatar Order Entry without OrderConnect

  • Install the latest myAvatar OE 2011 updates
