Medication Reconciliation section

This section presents a list of active medications that must be reconciled. Medication information populates from the following sources.

  • CCD Medications - Imported into myAvatar via one of the CCD import methods.
  • OrderConnect Prescriptions - Outpatient Prescriptions
  • OrderConnect Non-ISC Items - Patient Reported (OC)
  • Order Entry Orders - Inpatient Orders
  • Avatar Home Medication items - Patient Reported (CWS)

The goal of the reconciliation is to end up with one final list of active medications for the client. Use this section to consolidate multiple medication list items into one final list. Depending on the source of information for the medication different actions will be available. After actions are selected to be applied for the medications, the list of medications can be reconciled. A final review of medications and their corresponding actions is available.

Prerequisites:

To use the Medication Reconciliation section, you must be defined as a prescriber in either myAvatar Order Entry or OrderConnect, depending on which CPOE system you use. The following requirements must be met depending on the system in use:

  • Avatar Order Entry - You should be an active myAvatar  Order Entry user with prescribing rights. This is defined in the myAvatar  Order Entry User Definition, or Order Entry User Role forms. You must have rights to Create, Display and Discontinue Pharmacy Type Orders. Permission access should not be limited to ‘History Only Orders’.
  • OrderConnect - You should be an active OrderConnect user with prescribing rights.

The Allergy Reconciliation Grid Header contains the following information.

  • Reason for Reconciliation - User-defined dictionary that can be defined in Dictionary Update (File: CWS, Data Element #80200) that is used for data collection purposes only.
  • Transition of Care Record to Associate - Any reconciliation that is performed within two days of the Transition of Care will automatically be associated with the Transition of Care. If you want to link the Medication Reconciliation record directly to the Transition of Care, you can do so by using the Transition of Care Record to Associate field. This allows an agency to perform Medication Reconciliations past two days since the Transition of Care event was recorded. This is used during Measures Reporting.
  • Group Like Items - Select the Group Like Items field to group similar items in the Medications grid. Duplicate medications will be grouped together and highlighted in yellow.
  • Known Medications - This information is used for the Meaningful Use Measures reporting.
    • Yes will automatically be selected if there are active medications on record.
    • Select No if there are no active medications.  
  • Refresh Medications - Refreshes the Medication Reconciliation and Home Medication List sections of this form.
  • Select Staff to Notify - Notifies the selected staff member upon completion of Medication Reconciliation process.
  • Proceed with Filing - Selected after all actions you wish to proceed with are selected. This will present you with the Medication View Screen that will display all of the items that will be reconciled within the session.
  • View Final Active Problem List - Launches a view of all Active Medications assigned to the client.
  • Launch OrderConnect - Enabled after a reconciliation action has been performed in case you would like to go back into OrderConnect to make changes with items created.

The following links detail the available actions and results for each type of medication order that can be reconciled in this form.

Depending on the medication order type, and desired action, different results will take place. The links below list each available action, according to its medication order type.

Medication Reconciliation - Preview Grid

The Preview Grid is where all of the reconciliation work is determined. You can assign actions and New Medications to Add within this grid or discontinue existing items. This is the work list for you until you decide to Proceed with Filing and complete the Reconciliation process. 

Columns

  • Action
    • The available actions correspond to the source of information (displayed in the Source column).
    • Select the action to be applied. Depending on the action, specific fields will become enabled and required.
  • Type/Source
    • The Type and Source of Medication provide you with details to identify the correct source for the Medication item being display. Below is a table showing the Type and Source details.

  • Medication
    • Displays the medication description, dose, form and route of the medication item.
  • Type to Add
    • Select the type of medication to add. The available information types that can be added depend on the information source.
  • New Medication to Add
    • Enter the medication code. The available medications that can be selected depend on the information source.
  • Start Date
    • Displays the medication administration date.
  • Last Updated
    • Displays the last date the medication information was updated.
  • Details
    • Click View to display an overview of the medication information.
  • Practitioner
    • Displays the practitioner who is associated with the medication order.
  • Reconciliation

Duplicate medications rows are highlighted in yellow.

Reconciliation Steps

The following steps list the basic process for reconciling a medication order.

  1. In the Reason for Reconciliation field, select the reason for reconciliation.
  2. In the Transition of Care Record to Associate field, select the admission record that you want to link to the Medication Reconciliation. This is only necessary if you want to link the record because they occurred more than two days apart.
  3. Select the Group Like Items field to group similar items in the Medication grid.
  4. In the Known Medications field:
  • Select Yes if there are medications listed in the Clinical Reconciliation - Preview grid that are active.
  • Select No if there are no known medications for the client.
  1. Click Refresh Medications to update the view of medications in the Clinical Reconciliation - Preview grid to reflect any changes that have been made in the current session.
  2. In the Select Staff to Notify field, enter the practitioner that will be sent a notification when the medication reconciliation has been completed.
  3. In the Action column, select the action to perform. Depending on the source of the medication, different actions will be available and specific columns will become enabled and required.
    See the Medication Sources above for information on the medication source. Each medication source has different associated actions. 
  4. Click Proceed with Filing to open the Clinical Reconciliation - Preview screen.
  5. After medications have been reconciled (in the Clinical Reconciliation - preview screen), click View Final Active Medication List to view the Final Active Medication List screen.
  6. If appropriate, click Launch OrderConnect..
