Clinical Reconciliation forms

Versions

The Clinical Reconciliation version 1.0 form in myAvatar supports the ability to reconcile Problems, Allergies and Medications that are currently in the Patient’s chart as compared to data received from a CCD imported into myAvatar. This version was introduced with a lot of flexibility for medication reconciliation in that it supports the ability to create Order Entry Home Medications, Order Entry orders, OrderConnect Patient Reported Medications and OrderConnect prescriptions. 

In an effort to improve the workflow and simplify the process  Clinical Reconciliation version 2.0 has been introduced. User experience changes were made for clearer data presentation and actions available during the reconciliation process. In addition, this new Medication Reconciliation tab is geared towards Outpatient reconciliation. It will only support the creation of Home Medications. These are the patient reported medications either entered directly into OrderConnect or into the Home Medications tab of the OE Console.  
 

The Enable Improved Clinical Reconciliation Workflow registry setting implements several significant changes to the Clinical Reconciliation form. The following pages provide documentation on the features and functionality of this form before and after the implementation of improved workflow.
