Home Medication List section
The Home Medication List allows you to view and document a client’s Home Medication items. This includes both the patient-reported medications and any OrderConnect outpatient prescription. Each Home Medication item displayed or entered will need to be edited to capture the Medication Compliance status before proceeding to the Medication Reconciliation section of the form. All Home Medication items will populate in the Medication Reconciliation grid to provide the prescriber with the client’s entire medication regimen.
Prerequisites:
- The user accessing this form must be set up in myAvatar (User Definition form).
- The user must be associated with a practitioner (User Definition form, User Caseload section).
- If the facility is using myAvatar Order Entry the user must be set up in the Order Entry User Definition form.
- If the facility is using OrderConnect:
- The practitioner must be a prescribing practitioner in OrderConnect, with an associated NPI number.
- The practitioner must have an identical NPI, defined in the Practitioner Enrollment form.
This field is enabled through the Include NPI Number registry setting.
- The following information can populate in the Home Medication list:
- Active, non-InfoScriber orders entered in OrderConnect (non-ISC medications).
- Active ‘History Only’ orders entered in myAvatar Order Entry Order (Client Profile/Physicians Orders form, Create Order section, Order Category field).
- Medications that have passed their entered End Date will not show.
- Information can be entered into this table manually.
- You must edit each record for medication compliance before proceeding to the Medication Reconciliation section.
- All records entered in this section will populate in the Medication Reconciliation section.
Patient-Reported Medications
Current functionality allows patient-reported medications to be captured with the following methods:
- Non-ISC Medication in OrderConnect
- History Only Orders in myAvatar OE.
The History Only attribute is assigned in the Order Category field. - Home Medication List item
myAvatar OE users can enter patient-reported medications into the Home Medication List section manually.
Outpatient Prescriptions
All outpatient prescriptions are entered into OrderConnect and are considered part of the client's Home Medication List. All OrderConnect prescription will populate within the Home Medication List to capture compliance before proceeding with medication reconciliation.
- In the Home Medication List:
- Click Add New Item to create a new home medication record for myAvatar OE systems.
- Click Edit to edit details for a home medication.
- Click Delete to remove a home medication from the list for myAvatar OE systems. This is only supported on items that were added into the Home Medication List directly. You will be unable to remove either myAvatar OE History Only Orders or the OrderConnect Non-ISC items by using this function.
- In the Source field:
- Client/Family/Caregiver will be selected for myAvatar OE users when viewing History Only Orders and when adding a new Home Medication Item.
- The Order Entry Home Medication field utilizes the Order Code Search to allow you to capture patient-reported medications. Enter the Dosage, Units, Schedule, and Route of Admission fields.
- OC Reported will be selected for any Non-ISC medication entered in OrderConnect.
- OC Prescription will be selected for any OrderConnect Prescription from OrderConnect.
The OrderConnect Home Medication field populates with a display of either the OrderConnect Non-ISC reported item or the OrderConnect Prescription. You may enter information in the Dosage, Units, and Schedule fields, however, this information is usually contained within the medication description section. These fields will not be required when editing a Non-ISC item.
- In the Dosage, Units, and Schedule field, enter the medication information.
- In the Route of Administration field, select the medication route.
If Other is selected, enter the route information in the Describe Other Route field. - In the Additional Order Text field, enter information as appropriate.
- In the Home Medication Note field, enter any notes associated with the home medication order.
- In the Medication Compliance field, select the client's medication compliance. This is a required field for every entry that must be captured before proceeding to the Medication Reconciliation section.
In order for the medication item to populate the Medication Reconciliation section and be available for reconciliation, Taking as Prescribed must be selected. - In the Indication for use field, enter any medication indications.
- In the Last Dose Verified field, enter whether the last dosage could be verified.
- In the 'Last Dose' fields, enter the date and time of the last dosage verification.
- In the Item with Client/Family at Time of Admission field, select whether the medication was with the client or their family member at admission time.
If Yes is selected, enter the medication quantity in the Quantity Brought to Facility field. - In the Disposition of Item field, select the medication item disposition.
If Other is selected, enter the medication disposition in the Describe Other Disposition field. - Click Save Active List Entries.
