Clinical Reconciliation Version 2.0
Reconcile problems, allergies and medications that are imported into myAvatar via a CCD. These elements must first be imported into myAvatar and then reconciled with existing data potentially captured already in myAvatar .

- Development was introduced through the Alert User if Clinical Reconciliation Needed registry setting to provide a system notification when clinical reconciliation data is available for a client. When a CCD is associated to a client, myAvatar automatically parses the problems, allergies and medications into the Clinical Reconciliation temporary tables to be available for reconciliation.

Notification
- The External Source Information Available field indicates whether or not new external clinical information has been received.
- In the Reconciliation Type, select whether this is an Admission or General reconciliation.
- In the Episode field, select the specific client episode for the reconciliation. This is necessary for Meaningful Use Measures reporting purposes.
- In the Performed For field, search for and select providers that are defined as OrderConnect Prescribers.This field will allow you to conduct a reconciliation on a client for another provider.
General Workflow - Reconciliation Sections
There are three sections on the form where clinical reconciliation is performed. You will have the ability to reconcile problems, allergies and medications.
Here are general workflow considerations that are common between all sections:
- Any CCD Imported item will be highlighted in yellow for easy identification.
- Double-clicking on any column will sort the list by that column.
- The View button will show details of the record.
- The Practitioner and Reconciliation columns will show the last date of reconciliation and the practitioner who performed it. this will populate with the practitioner selected within the Performed For field, if used by the date entry user.
- The Known Problems, Known Allergies and Known Medications will automatically populate to Yes if there are records already defined in myAvatar or OrderConnect.
- The second column within the reconciliation grids will allow you to select there cord via a checkbox. Select items you wish to take action on and then select the action button below the screen.
The action will appear in the Include in Record column. - For faster processing, you can select the Do Not Include Remaining button to select that action for the rest of the records.
- The Start Date and Last Update date provided within the CCD will default into the reconciliation grids if provided. These dates will be used when adding records within myAvatar via reconciliation.
- CCD Imported records will generally automatically match to the RxNorm or SNOMED database within myAvatar . If this happens then the New Problem to Add, New Allergy to Add or New Medication to Add columns will be auto-populated with the corresponding problem, allergy or medication that can be added if you wish to do so. This will streamline the reconciliation process as you can skip the step to search for a match in myAvatar to add.
- Review Final List is the final step to review everything selected for reconciliation. You can choose to remove items you do not wish to reconcile at that time, before proceeding with reconciliation.
