Client Charge Input With Diagnosis Entry - PM

Enter services (charges) for a client.

Prerequisites:

  1. Go to: Avatar PM > Services > Ancillary/Ambulatory Services > Client Charge Input With Diagnosis Entry.
  2. In the Date Of Service field, enter the service date.
  3. In the Client ID field, enter the client name or ID, and select.
  4. In the Episode Number field, select the client episode.
  5. In the Program field, select the client program.
  6. In the Service Code field, enter the service code name or number, and select.
    Note: The service must be required by a client, or a client and practitioner (Service Required By field, Service Codes form).
  7. In the Practitioner field, enter the practitioner name or ID, and select.
  8. Click Display Managed Care Authorization Data to display managed care authorizations for the client, service code, and date of service.
  9. In the Duration (Minutes) field, enter the service duration.
  10. In the Location field, select the service location.
  11. In the Co-Practitioner field, enter the practitioner name or ID, and select
  12. The Cost of Service field displays the service fee, set up in the Service Codes, and Service Fee/Cross Reference Maintenance forms.
  13. In the 'Active Diagnosis' fields, select the medical diagnosis. This list contains medical diagnoses from the Diagnosis record based upon service date.
  14. In the 'Diagnosis' fields, enter the ICD-10 code, and select.
  15. In the Additional Service Information field, select the applicable codes.
  16. In the Psychotherapy Add-On Duration field, enter the add-on duration in minutes.
  17. In the Add-On Notes field, enter any additional information related to the add-on.
  18. Click Submit.
