Client Charge Input - PM
Enter services (charges) for a client with no associated clinical documentation. Submitting this form posts the service fees to the client ledger and adds the services to the client’s treatment history.
The Cost Of Service field can be edited in the Client Charge Input (Charge Fee Access) form and the Client Charge Input (Charge Fee Access and Diagnosis Entry) form.
Optional Prerequisites:
- A managed care authorization must be set up for the client in the Managed Care Authorizations form.
- Go to Avatar PM > Services > Ancillary/Ambulatory Services > Client Charge Input.
- In the Date Of Service field, enter the service date using one of these options: enter the date in MMDDYYYY format, select the calendar icon and choose a date, or select the T (today) or Y (yesterday) button.
Note: The date may default to the current date. - In the Service Start Time field, enter the time in the HH:MM AM/PM format, or select the Current Time button.
- In the Service End Time field, enter the time in the HH:MM AM/PM format, or select the Current Time button.
- In the Client ID field, enter and select the client name or ID for the client for whom you are entering services.
- In the Episode Number field, select the client episode the service refers to.
- In the Program field, select the program that the service was performed under.
Note: The program defaults to the program associated with the client episode selected in the Episode Number field. If there are multiple programs associated with the episode, select the applicable program for service entry. - In the Service Code field, search for and select the service code name or number associated with the service provided.
Note: The services available for selection must be associated with the program selected in the Program field. - In the Original Scheduled Service field, search for the original scheduled service, if applicable.
- In the Modifiers field, enter the appropriate modifier(s).
Notes:- You can enter multiple modifiers separated by a comma or a colon.
- Modifiers entered in this form override modifiers normally generated by a service fee cross reference.
- In the Practitioner field, select the name or ID of the practitioner who provided the service.
Note: Practitioners available for selection must be associated with the program selected in the Program field. - Select Display Managed Care Authorization Data to display managed care authorizations for the client, service code, and date of service. The data appears in a new window.
Note: To display manage care authorization data, an authorization must be entered in the managed care authorization prior to using the Client Charge Input form. - In the Location field, select the service location.
Note: The Location field automatically populates as the location where the program selected in the Program field is located, if the location is set as the default for that program or only one location is associated with that program. If multiple locations are associated with the program of service, you need to select a location. - In the Duration (Minutes) field, enter the duration of the service provided.
Note: If the Service Start Time and Service End Time fields are populated, the Duration is automatically populated with the number of minutes between the Service Start Time and Service End Time. - In the Co-Practitioner field, search for and select the name or ID of the Co-Practitioner, if applicable.
- The Cost of Service field automatically populates a cost based on the service code entered in the Service Code field.
Note: Set up and define the cost of service in the Service Codes and Service Fee/Cross Reference Maintenance forms. - In the Referring Practitioner field, search for and select the name of the referring practitioner if the client was referred.
- In the Referring Provider field, search for and select the name of the referring provider if the client was referred.
- In the Date of Accident (2300-DTP-03) field, if the client is being treated and charged due to an accident, enter the date using one of these options: enter the date in MMDDYYYY format, select the calendar icon and choose a date, or select the T (today) or Y (yesterday) button.
- In the Related Causes Information (2300-CLM-11) field, select the causes of the accident.
Note: This field becomes required when a date is entered in the Date of Accident field
- In the Form Locator 10 (Auto Accident, Place) field, select a form.
Note: This field is enabled and required when Auto Accident is selected in the Related Causes Information (2300-CLM-11) field. - In the Add-On Service field, select an add-on service, if applicable.
Note: The service code must be designated as an add-on code for it to be used in this field. - In the Add-On Duration field, enter the duration of the add-on service in minutes if applicable
- In the Add-On Notes field, enter any necessary clinical notes for the add-on service.
- Select Save Add-On Service to save the information about the add-on service.
- Optional. If you need to edit or remove a saved add-on service, select the add-on service in the Select Add-On Service Entry to Edit/Remove field.
Note: The add-on service selected in the Select Add-On Service Entry to Edit/Remove field displays in the Selected Add-On Services field to either edit or remove. - Optional. If removing a service, select the Remove Add-On Service to remove the service.
- In the Co-Practitioner Duration (Minutes) field, enter the duration of the co-practitioner's service, if applicable.
- Select Submit.
