Update Client Data - PM

Manage basic demographic information for a client.

• When this form is submitted, client’s demographic information is updated throughout myAvatar PM/Cal-PM.
• The information from Demographics section of the Admission form populates this form and can be edited.

1. Go to: Avatar PM > Client Management > Client Information > Update Client Data
2. In the Select Client screen, enter the client ID in the Client Name/ID field, select the Search button, and select the corresponding entry.
   Note: Information entered in the Pre Admit form, and the Demographics section (Admission form) populates the associated fields, and can be edited.
3. Provide demographic client information. 
4. In the Smoking Status Assessment Date field, enter the date the smoking status was taken.
5. In the Client Declined To Provide Information On The Following field, select the categories which the client chose not to disclose.
6. In the Alias fields, enter client alias as appropriate
7. In the Mother's Maiden Name field, enter the mother's maiden name for identification purposes.
8. In the Protection Indicator field, select whether or not the client's information may be shared with others. 
   Note: If Yes, enter the date that this protection should begin in the Protection Indicator Effective Date field.
   These fields are for immunization reporting and are used to populate the publicity code and publicity code effective date respectively (HL7 segment PD1, fields 11 and 18).
9. In the Name Qualifier field, select whether or not there are restrictions on disclosing the client's name.
   Note: This field is for syndromic reporting and is used to determine if the client's name should be included in the HL7 message (HL7 segment PID field 5).
10. Select Submit.
    Note: The Import Client Picture link below the Select button opens the Import Client Picture form (RADPlus Utilities).
