Demographics Section - PM

Enter client demographic information.

This section is available on the following forms:

• Admission
• Admission (Outpatient)
• Call Intake
• Discharge
• Pre Admit
• Pre Admit Discharge

1. Enter client demographics. The following information is maintained in the Client file of the Dictionary Update form:
   • Gender Identity
   • Sexual Orientation
   • Client’s Address - County
   • Primary Language
   • Client Race
   • Ethnic Origin
   • Religion
   • Country of Origin
   • Marital Status
   • Education
   • Employment Status
   • Occupation
   • Pregnancy Status
2. In the 'Alias' fields, enter client alias information as appropriate.
3. In the 'Mailling Information' fields, enter address information as appropriate.
4. For the Smoker field, enter the client's smoking status.
5. In the Smoking Status Assessment Date field, enter the date the smoking status was taken.
6. In the Client Declined To Provide Information On The Following field, select the categories that the client chose not to disclose.
7. In the Pregnancy Status field, select the client's status. This field is enabled when Female is selected in the Sex field.
8. In the Expected Due Date field, enter the anticipated date of delivery. This field is enabled when Female is selected in the Sex field.
9. In the Lactating Status field, select whether or not the client is lactating. This field is enabled when Female is selected in the Sex field.
10. In the Mother's Maiden Name field, enter the mother's maiden name for identification purposes.
11. In the Protection Indicator field, select whether or not the client's information may be shared with others. If Yes, enter the date that this protection should begin in the Protection Indicator Effective Date field.
    These fields are for immunization reporting and are used to populate the publicity code and publicity code effective date respectively (HL7 segment PD1, fields 11 and 18).
12. In the Name Qualifier field, select whether or not there are restrictions on disclosing the client's name. This field is for syndromic reporting and is used to determine if the client's name should be included in the HL7 message (HL7 segment PID field 5).
13. Click Submit.
