Update Service Medical Diagnosis - PM

Manage medical diagnoses associated with rendered services.

Prerequisites:

• Services are available for selection in this form, when Yes is selected for the treatment program (Primarily Medical Program field, Program Maintenance form).
• The service must be required by a client, or a client and practitioner (Service Required By field, Service Codes form).

1. In the Client ID field, enter the client name or ID, and select.
2. In the Episode Number field, select the client episode.
3. In the Service Begin Date field, enter the earliest service date to display in the Service Selection screen.
4. In the Service End Date field, enter the latest service date to list in the Service Selection screen.
5. Click Select Service(s) To Update Medical Diagnosis to display the Service Selection screen. Use this screen to associate medical diagnoses with services.

• Rows display a date of service.
• Select a row. To select multiple rows, drag the mouse from row to row, or select a row and hold the Shift key.
• Click Ok.

6. In the Service Selection Criteria field:

• Select All Services to display all services in the Service Selection screen.
• Select Only Services Missing Diagnosis to display services without a medical diagnosis in the Service Selection screen.
• If dates are not provided, the Service Selection screen displays all posted services (or all posted services missing diagnosis).

7. In the 'Active Diagnosis' fields, select the medical diagnosis. This list contains medical diagnoses associated with the client in the Medical Diagnosis form.
8. In the Medical Diagnosis 1 and 2 fields, enter the ICD-9 code, and select.
9. Click Update Service(s).
10. Click Submit.
