Discontinue Care Plan for patient

Path: Patient>Clinical>Care Plans

When discontinuing a care plan, you discontinue all the goals and interventions associated with this care plan. You may also need to enter health status evaluation for the open goals. To discontinue a Care Plan for a patient, complete the following steps.

  1. In the Patient component, select a patient.
  2. Go to Clinical > Care Plans.
  3. In the left panel, click + next to Active to display care plans currently assigned to the patient.
  4. Select the care plan you want to discontinue.
  5. For each goal and intervention associated with the care plan, enter the date when it is discontinued in the End Date field. 
    Tip: To discontinue all open goals and interventions for a care plan at once, click Discontinue in the top right corner and complete the fields in the Discontinue
  6. If needed, enter the health status evaluation for each goal.
    Note: The Health Status Evaluation field is available if your Business Unit is set to require it (in Administration > Configuration > Business Units > Settings > Clinical Miscellaneous) and the appropriate privilege is granted.
  7. If the Y value is selected for the goal/intervention in the O (Generate Order Option) column, specify the ID of physician who ended the goal or intervention. For the selected physician, the supplemental order is generated.
