Client Header
The Client Header displays when you access a form for a specific client or the client chart. The information displayed will depend on which form you are viewing, which client is open, and which client episode is open. Information which has not been entered into myAvatar for the client will be omitted.

The image above is a sample of a Client Header. This information may display differently, depending on your facility's setup.
The first section of the Client Header displays the following client demographic information and a client picture, if one has been associated with the client.
- Name
- Client ID
- Sex
- Age
- Date of Birth
- Height
- Weight
- BMI
The second section displays two columns of episode-specific information. These columns appear only when viewing forms or data pertaining to a selected episode.
- Episode (number and program)
- Problem Code
- Diagnosis Code
- Location (unit / room / bed)
- Attending Practitioner
- Admitting Practitioner
The third section displays Client Alerts pertaining to the form currently being viewed. When viewing the Client Chart, this section displays all active alerts.
The fourth section displays CareGuidance alerts and client allergies, if allergies have been associated with the client.
- If there are known allergies, entered through the Allergies and Hypersensitivities form in myAvatar CWS, these allergies display in red.
- If there are any unacknowledged CareGuidance alerts, these display underneath the allergies.
The row beneath the Client Header displays user-selectable widget buttons that, when selected, open a modal to display the selected widget.
- The header widget allows up to 5 widget buttons.
- Widget buttons for NX Client Header are configured using the Client Lookup/Header Configuration Manager form.
