Demographics Section
Enter client demographic information in the Demographics Section.
Note: Information entered in the Pre Admit, Pre Admit Discharge, Admission, Admission (Outpatient), and Discharge forms populates the Demographics fields. This information can be edited.
- In the Address fields, enter the client's address information.
- See Client’s Address - County for values for this field.
- In the Communication Preference field, select the method by which the client prefers to be contacted, if applicable.
- Select the client's Primary Language.
- Select the Client Race.
- If additional races apply, select which ones in the Other Race(s) field.
- Select the client's Ethnic Origin.
- In the Client Declined To Provide Information On The Following field, select the categories which the client chose not to disclose.
- Select the client's professed Religion.
- In the Place Of Birth field, enter the location where the client was born.
- Select the client's Country of Origin.
- Select the client's Marital Status.
- In the Maiden Name field, enter the client's unmarried surname, if applicable.
- Select the client's level of Education.
- Select the client's current Employment Status.
- Select the client's Occupation.
- In the Are you heterosexual, lesbian, gay, bisexual, transgender or do you question your sexual orientation? field, select the client's self-identification.
- The value reported in this field is reported in a Cal-OMS Submission file in field CID-20.
- To enable this field, select Yes for the Program Maintenance field Require Sexual Orientation Response During Admission?
- Select the client's level as a Smoker.
- In the Smoking Status Assessment Date field, enter the date the smoking status was taken.
- In the 'Alias' fields, enter client alias information as appropriate.
- In the Mother's Maiden Name field, enter the mother's maiden name for identification purposes.
- In the Protection Indicator field, select whether or not the client's information may be shared with others. If Yes, enter the date that this protection should begin in the Protection Indicator Effective Date field.
These fields are for immunization reporting and are used to populate the publicity code and publicity code effective date respectively (HL7 segment PD1, fields 11 and 18). - In the Name Qualifier field, select whether or not there are restrictions on disclosing the client's name.
- This field is for syndromic reporting and is used to determine if the client's name should be included in the HL7 message

- In the Pregnancy Status field, select whether or not the client is pregnant.
- This is a dictionary field that accepts user defined codes.
- It is used for reporting purposes, to communicate information to RXConnect and does not have an impact on billing.
- In the Expected Due Date field, if the client is pregnant, define the expected date of delivery given by their OB-GYN.
- In the Lactating Status field, select Yes if the client is actively breast feeding.
