STD History

In the STD module, the STD History form is where you record the patient's history of sexually transmitted diseases, including syphilis.

On the main menu navigate to History > STD History.

STD history can be added but not edited on this form.

Diagnoses added on the STD History form will appear in the following places in the STD module.

Change in STD history

If there is no change in the patient's STD history, for the Any change item at the top of the form, select No.

Add STD history 

  1. Below the STD History panel, click Add Entry.
  2. In the Search Diagnosis box, either enter all or part of the diagnosis name or all or part of the ICD-9 or ICD-10 code. Click Search.
  3. In the search results, select the diagnosis. The diagnosis will appear in the Selected Diagnosis field.
  4. The Onset Date defaults to today's date and cannot be changed. 
    Note: To clear the entry and start over, click Reset. To discard the entry, click Cancel.
  5. Click Save Entry.

Add syphilis history

  1. Below the Syphilis History panel, click Add Entry.
  2. In the Non-treponemal testing section, for each of the following items, click the arrow and select the appropriate choice.  
    • Non-treponemal test type
    • Non-treponemal test result
    • Non-treponemal titer
  3. In the Treponemal testing section, for each of the following items, click the arrow and select the appropriate choice.
    • Treponemal test type
    • Treponemal test result
  4. In the Date box, enter the date for the testing. Optionally click the previous and next arrows or click the calendar icon to select a date. To clear the date, click the X.
  5. For each of the following items, click the arrow and select the appropriate choice.
    • Direct testing type
    • LP-CS VDL
    • Stage
    • Treatment

Note: To clear the fields and start over, click Reset. To discard the entry, click Cancel.

  1. Click Save Entry.  

Titer history

  1. For the See titer history item, select Yes to indicate the titer history should be reviewed.
  2. In the Titer History Last Updated On box, enter the date. Optionally click the previous and next arrows or click the calendar icon  to select a date. To clear the date, click the X.
  3. On the toolbar above the menu, click the Save icon.
