Diagnosis - STD module
In the STD module, the Diagnosis form is where you add STD and non-STD diagnoses to the patient's diagnosis history and enter specimen collection and CMR submission information for the CMR Confidential Morbidity Report (CMR).
On the main menu click Diagnosis.
- The information entered on the Diagnosis form is saved in OrderConnect. Information for existing diagnoses is retrieved from OrderConnect.
- The end date cannot be added for a diagnosis. This feature ensures that the diagnosis information is available in OrderConnect
- STD and non-STD diagnoses entered on the Diagnosis form will appear in the following places in the STD module.
- History > STD History form (STD History grid)
- History > Med/Surg History form (Diagnosis History grid)
- Treatment > Meds/Procedures form > Treatment Submission (Diagnosis grid)
Add an STD diagnosis
- Below the STD Diagnosis grid, click Add Entry.
- In the Search Diagnosis box, enter the first letter or letters of the diagnosis name or the ICD-9 or ICD-10 code and click Search. In the list, click the diagnosis, which then appears in the Selected Diagnosis box.
- For the Onset Date, enter the date. Optionally click the arrows or click the calendar icon to select a date.
- Click Save Entry.
Add a non-STD diagnosis
- Below the Non-STD Diagnosis grid, click Add Entry.
- In the Search Diagnosis box, enter the first letter or letters of the diagnosis name or the ICD-9 or ICD-10 code and click Search. In the list, click the diagnosis, which then appears in the Selected Diagnosis box.
- For the Onset Date, enter the date. Optionally click the arrows or click the calendar icon to select a date.
- Click Save Entry.
Lab diagnosis
The Lab Diagnosis is for display only and is the diagnosis selected on the In-Clinic Labs form.
Confidential Morbidity Report (CMR)
- Below the CMR grid, click Add Entry. A Diagnosis window appears that includes all of the patient's STD and non-STD diagnoses from the preceding sections of the Diagnosis form.
- Select a diagnosis from the list and the diagnosis appears in the Diagnosis field.
- For the Date of Diagnosis, Date of First Specimen Collection, and Date of CMR Submission fields, enter a date for each one. Optionally click the arrows or click the calendar icon to select a date.
- Click Save Entry.
