Add Treatment Setup Screen

In the CPT Code field, select the CPT or revenue code.

Select the clinician.
- Clinicians associated with the contracting provider display.
- The clinician's enrollment start and end date (if any) display.

Select the performing provider's license type.

Enter the number of service units per day.

Select the program associated with the treatment.

Enter the date, or dates of service.

Click Set Treatment Date.
- The CPT or revenue code fee definition is retrieved from Avatar MSO.
- The Add Treatment - Details screen displays.
