The Facility field displays the facility number associated with the treatment program.

Select a treatment setting.
- The Episode Number fields displays episode 1 for the new client.
- The Treatment Service field displays the treatment service associated with the treatment program.

Select the following:
- Type of admission, Source of admission, admitting practitioner.
- The Age field displays the client’s age calculated from their date of birth.

Enter the admission date in the format m/d/yyyy.

Enter the admission time in the format HH:MM AM/PM.

Select the treatment program.
For Cal-PM systems, the treatment program field is populated by only those Avatar Cal-PM programs that are mapped to the user's contracting provider (in Avatar MSO) and under which the member does not also have an existing Admission/Discharge episode existing in Avatar PM and ProviderConnect.

Select the practitioner type.
- The RRG field displays the RRG extended dictionary value for the program in the Program field.

Select whether to perform a discharge alert.
- If Yes, select the type of alert.

Select whether to allow visitations.

Select the following:
- Reason for contact, accompanying person relationship.
- Client's disposition, Client's custody status, presenting problems.
