Treatment dates of service

Single Date
Enter the date in the format m/d/ yyyy

Date Range
Enter a range of dates in the format m/d/ yyyy - m/d/yyyy
For example 1/1/2009 - 1/10/2009
If Include Weekends is selected, the date range can include service performed on weekend dates.

Multiple Dates
Enter multiple individual dates of service.
Enter dates manually or click Calendar to select individual dates. Dates turn yellow when selected. Click the dates, click Select Dates.
