Record details of the client's psychosocial history.

  1. In the Client Psychiatric History field, enter any relevant details regarding the client's previous psychiatric state.
  2. In the Client Medical History field, enter any relevant details regarding the client's previous medical condition.
  3. In the Client Substance Abuse History field, enter any relevant details regarding the client's previous relationship with substance abuse.
  4. In the Past / Present Biopsychosocial Functioning field, enter any relevant details regarding the client's biopsychosocial state.
  5. In the Developmental History (Physical, Psychological, Social) field, enter any relevant details regarding the client's development.
  6. In the Physical field, select whether or not the client has experienced physical abuse.
    Note: If Yes, enter any relevant details in the Physical Comments field.
  7. In the Sexual field, select whether or not the the client has experienced sexual abuse.
    Note: If Yes, enter any relevant details in the Sexual Comments field.
  8. In the Emotional field, select whether or not the client has experienced emotional abuse.
    Note: If Yes, enter any relevant details in the Emotional Comments field.
  9. In the Family Constellation field, enter any relevant details regarding family constellation therapy.
  10. In the Marriage / Significant Other field, enter any relevant details regarding the client's relationship with their significant other. 
  11. In the Children field, enter any relevant details regarding the client's relationship with their children.
  12. In the Family Psychiatric, Medical and Substance Abuse History field, enter any relevant details regarding a history of abuse in the family.
  13. In the Past / Present Relationship with Family / Significant Other field, enter any relevant details regarding the client's relationship with family members.
  14. In the Military Service field, select whether or not the client has served in any branch of the military.
    If Yes, enter any relevant details in the Military Service Details field.
