Client Health Maintenance

Display immunization and wellness management alerts for a client. From the Other Chart Entry form you can:

  • Add immunizations given by the facility, or by an outside provider.
  • Screen for immunization and allergy interactions.
  • Create links to view vaccine guidelines.
  • Create a report of the client’s immunization and wellness history including current alerts
  1. Go to: Avatar CWS > Other Chart Entry
  2. In the Client ID field, enter the client ID, click Select Client Name/ID Search. Click OK.
  3. The Alerts field displays immunization and wellness item alerts, items that are due, past due, or will be due in upcoming months (defined in the 'Include Alerts X Number Of Months In Advance' registry setting).
  4. In the Immunizations fields:
    • Click the List Immunizations button to display a report that lists the client's immunizations, immunization alerts that are due, past due, or will be due in upcoming months.
    • Click the Update button to display the Immunizations screen.
      • Immunizations Screen
        • A new row will appear upon entering the immunizations field.
        • In the Vaccine column, double-click and search by the code/description. (i.e. 82/Adenovirus)
        • In the Dose field, enter the appropriate dosage. (i.e. 1)
        • In the Provide By column, select a provider. (i.e. 1/Facility)
        • In the Date column, enter the date of immunization.
        • If applicable enter the appropriate information for the Refused, Reason, Time, Amount, Unit, Unit-Other, Route, Route-Other, Site, Body Site-Other, NDC, Manufacturer, Lot #, Exp Date, Ordered by, Administered By, Administration Comments, Source of Immunization, Vaccine Statement Shown to Patient, Date Vaccine Statement Presented to Patient, Name of VIS Statement, VIS Number, VIS Edition Date, VIS Edition Status, and VIS Last Updated Date columns.
        • Click the Save button.
    • Click Screen to screen the immunization that are due or are past due for allergy interactions. If an interaction is found, a message displays for the user to enter an override reason.
      This field displays when immunization screening is enabled in the Health Maintenance Guideline Definition - Facility form.
    • Click View CDC Immunization Guidelines to navigate to the http://www.cdc.gov/vaccines/ website, for vaccine guidelines.
  5. In the Wellness fields:
    1. Click the Update button to display the Wellness Management screen.
      • Wellness Management Screen
        • In the Wellness Item column, double-click and search by the code/description. (i.e. 5/Meditation)
        • In the Provided By column, double-click and select the provider. (i.e. 1/Facility)
        • In the Date column, enter the date of the wellness item.
        • if applicable, enter the appropriate information for the Comments, Refused, and Administered By fields.
        • Click the Save button.
    2. Click the List Wellness button to display a report that lists the client's wellness items, wellness item alerts that are due, past due, or will be due in upcoming months (defined in the Health Maintenance Alert registry setting)
  6. In the Publicity Code field, select the applicable code.
  7. In the Publicity Code Effective Date field, select a date.
  8. In the Immunization Registry Status field, select a status
  9. In the Immunization Registry Effective Date field, select a date.
  10. Click the Query Immunization Registry button. 
  11. Once finished, click the Submit button.
