Client Treatment Plan

When you create a treatment plan for a client using this form you can complete each section individually. This form works with the Treatment Plan Definition form and data in the fields can be sent from a myAvatar CWS assessment.

When a new treatment plan is created, the Client Treatment Plan section must be filed before information can be entered in any other section.

A client treatment plan's status can be changed to draft in the Treatment Plan Status Override form.

Prerequisites:


  1. Go to: Avatar CWS > Treatment Planning > Client Treatment Plan
  2. On the Select Client screen, search for and select the client.
  3. If the client has multiple episodes, choose the episode from the Select Episode list.
  4. In the Plan Date field, enter the treatment plan date.
  5. In the Plan Name field, type the treatment plan name.
  6. In the Plan Type field, select the plan type.
  7. Select Library to choose a library.
    Note: Libraries are defined in the Treatment Plan Definition form.
  8. If appropriate, in the Select Library field, choose a library to choose problems, goals, etc. when creating the treatment plan.
  9. Select Items For Plan to choose items (problems, goals, etc.) from the library selected in the Select Library field.
    Note: In the TX Plan Selections screen, select the goal.
  10. Select Ok.
    Note: Once selected these items can be edited.
  11. Select Print Client Plan to view a report of the treatment plan information.
  12. In the Treatment Plan Status, select the status.
    Note: If the user is on a Client Treatment Plan Required list, Draft and Pending Approval are available. If the user is not on a required list, Draft and Final are available.
  13. If appropriate, in the Team Member To Notify field select the team member.
    Note: Team members are assigned in the Notification Users form (RADplus Utilities).
  14. Once finished, select Submit.
