Edit Service Information - PM

Edit service information for multiple posted services.

Note: If the service being edited is associated with a progress note that generated a document, that document is appended with the new service information upon filing. Additionally, a To-Do item will be sent to the service practitioner to inform them about the change.

Prerequisites:

  • Services are available for selection in this form, when Yes is selected for the treatment program (Primarily Medical Program field, Program Maintenance form).
  • The service must be required by a client, or a client and practitioner (Service Required By field, Service Codes form).
Select Service
  1. Go to Avatar PM > Services > Ancillary/Ambulatory Services > Edit Service Information.
  2. In the Client ID field, enter the client name or ID, and select.
  3. In the Episode Number field, select the client episode.
  4. In the Service Start Date field, enter the earliest service date to display in the Select Service(s) To Edit screen.
  5. In the Service End Date field, enter the latest service date to list in the Select Service(s) To Edit screen. If no dates are entered, the Select Service(s) To Edit screen displays all available services.
  6. In the Claim Number field, select the claim number to filter services by.
  7. In the Service Selection Default field:
    • Select All to have all services chosen in the Select Service(s) To Edit screen.
    • Select None to have no services selected.
  8. Click Select Service(s) To Edit to display the Select Service(s) to Edit screen. Select the services, click Ok. Different services and dates of service can be selected.
Service Details

The Service CodeProgram, and Duration (Minutes) fields can be edited for services with an OPEN billing status. These fields are display-only for services with a CLOSED billing status.

  1. In the Service Code field, enter the service code name or number, and select.
  2. In the Program field, select the program.
  3. In the Location field, select the location.
  4. In the Duration (Minutes) field, enter the service duration.
  5. The Cost of Service field is display-only and shows the service fee.
Practitioner
  1. In the Practitioner field, enter the practitioner name, click Process Search to select. Click Ok.
  2. In the Co-Practitioner field, enter the practitioner name or ID, and select.
  3. In the Second Co Staff Member field, enter the co staff member name or ID, and select.
  4. In the Modifiers field, enter the modifier codes separated by commas without spaces.
  5. In the Service Start Time and End Time fields, enter the time frame of the service.
  6. In the Referring Practitioner field, search for and select the referring practitioner. 
  7. In the Referring Provider field, search for and select the referring provider. 
  8. In the Emergency Indicator field, select Yes or No. 
  9. In the 'Diagnosis' fields, enter the medical diagnosis, and select. Medical Diagnosis fields cannot be edited for posted services.
  10. If applicable, select a claim number in the Claim Number field. 
  11. In the Co-Practitioner Duration (Minutes) field, enter the duration. 
Additional Data
  1. In the Add-On Service field, select a service. After selection, click the Save Add-on Services button.
  2. To remove or edit an add-on service select a service in the Select Add-on Service Entry to Edit/Remove field. Click Remove Add-on Service button to remove the selected service.
  3. In the Evidence Based Practices field, select the desired value(s). 
  4. In the Form Locator 10 (auto accident, place) field, select the desired value(s). 
  5. In the Related Causes Information (2300-CLM-11) field, select the desired value(s). 
  6. In the Date of Accident (2300-DTP-03) field, enter the date. 
  7. Select a discipline in the Discipline field. 
  8. In the First Billing Date field, enter the date.
  9. In the Initial Treatment Date (2300-DTP-03) field, enter the date. 
  10. In the Original Scheduled Service field, search for and select the original service. 
  11. In the Service Status field, select the value(s).
  12. In the Additional Service Information field, select the add-ons for this service. These values are set up in the Service Codes form.
    • If Interactive Complexity is present during the service, select Interactive Complexity to have the system also render the Interactive Complexity service code associated to the service code. This is only available for service codes that are defined as Evaluation ManagementPsychotherapy, or Group Therapy from the service code setup.
  13. In the Psychotherapy Add-On Duration field, enter the additional time frame.
    • If psychotherapy is present during the service, select Psychotherapy Add-On to have the system also render the Add-On Service Code associated to the service code. This is only available for service codes that are defined as Evaluation Management from the service code setup.
  14. In the Add-On Notes field, enter any applicable notes.
  15. Click Submit.
