Patient

Patient's name entered in Patient>General>Basic.

DOB

Patient's date of birth entered in Patient>General>Basic.

Service

Code and description of a service within which visit notes were documented.

Date of service

Date of the service within which visit notes were documented.

Admit Date

Patient's admission date entered in Patient>General>Admissions & Status.

Cert Period

Start and end date of the certification period within which the service was performed.

Team

Team to which a patient belongs, if any. If you generate the report in the Reports component, it is shown only if the Show Team on Report check box is selected on the Define tab.

Team is available only if your agency is set up to use teams at the Business Unit level. Teams are associated with a patient in Patient>General>Admissions & Status.

Branch

Branch to which a patient belongs, if any. If you generate the report in the Reports component, it is shown only if the Show Team on Report check box is selected on the Define tab.

Branch is available only if your agency is set up to use branches at the Business Unit level. Branches are associated with a patient in Patient>General>Admissions & Status.

Homebound Status

Reason why a patient is homebound entered in Patient>Clinical>Visit Notes.

Signature

Name of the resource providing visit notes and date of service within which visit notes were documented.
