Quality Measures report- Comfort Level

The Comfort Level section of the Quality Measures report shows the patient's comfort level information.

At the top right corner of the report the following information is printed: Business Unit, reporting period, branch and team (if any), whether 24 hours data is shown. If the report shows patients from multiple teams and branches, the patients' information is sorted first by team, and then by branch.

The following table describes the fields of this part of the report.

Field Description

Team

Team to which a patient belongs, if any. Patient team is available only if your agency is set up to use teams at the Business Unit level. Teams are associated with a patient in Patient>General>Admissions & Status.

Branch

Branch to which a patient belongs, if any. Patient's branch is available only if your agency is set up to use branches at the Business Unit level. Branches are defined in Administration>Configuration>Business Units>Branches.

Patient Code

Patient's ID entered in Patient>General>Basic.

Patient Name

Patient's name entered in Patient>General>Basic.

Primary Physician

First and last name of the patient's primary physician.

Admission Date

Patient's admission date entered in Patient>General>Admissions & Status.

Discharge Date

Patient's discharge date, if any, entered in Patient>General>Admissions & Status.

Primary Diagnosis

Patient's primary diagnosis entered in Patient>General>Diagnoses.

Date Provided

Date when the comfort level was checked and provided.

Episode

Type of the episode during which the comfort level was assessed (Initial Assessment or Pain Episode) entered in Patient>Clinical>Quality Measures.

Self-Reported

Indicates if a patient was able to self-report the comfort level, entered in Patient>Clinical>Quality Measures.

Uncomfortable

Indicates if a patient was uncomfortable during the comfort level assessment, entered in Patient>Clinical>Quality Measures.

Indicates if a patient was able to self-report the comfort level in 24 and 48 hours after the initial assessment, entered in Patient>Clinical>Quality Measures.

Comfort Level
in 24 hrs

Indicates if the patient's pain was brought to a comfortable level in 24 and in 48 hours, entered in Patient>Clinical>Quality Measures.

Reason why unable to self- report
in 24 hrs
in 48 hrs

Reason provided if a patient could not self-report during the first or subsequent comfort level assessments, entered in Patient>Clinical>Quality Measures.
