Client Observation - 'cw_observations_detail_archive' SQL table
Note - These testing guidelines assume the user is skilled in the use of, at a minimum, the following:
  • Change MR#
Scenario 1: Client Merge - Validate the 'cw_observations_detail_archive' SQL table
Specific Setup:
  • A client must be enrolled in an existing episode and have 'Client Observation' records on file from 30+ days ago that display in the 'cw_observations_detail_archive' table (Client A).
  • A second client must be enrolled in an existing episode (Client B).
Steps
  1. Access Crystal Reports or other SQL Reporting tools.
  2. Select the CWS namespace.
  3. Create a report using the 'cw_observations_details_archive' SQL table.
  4. Navigate to the existing row for the record for "Client A".
  5. Validate the 'PATID' field contains the ID for "Client A"
  6. Access the 'Client Merge' form.
  7. Select "Client A" in the 'Source Client' field.
  8. Select "Episode 1" in the 'Source Client Episode' field.
  9. Select "Client B" in the 'Target Client' field.
  10. Click [File].
  11. Validate a confirmation message is displayed stating: "Do you wish to continue with the indicated action?".
  12. Click [Yes].
  13. Validate a confirmation message is displayed stating: "The following new episode has been created for the target client indicated. Episode 2"
  14. Click [OK] and close the form.
  15. Access Crystal Reports or other SQL Reporting tools.
  16. Refresh the report using the 'cw_observations_details_archive' SQL table.
  17. Navigate to the existing row for the record for "Client A".
  18. Validate the 'PATID' field now contains the ID for "Client B".
  19. Validate the 'EPISODE_NUMBER' field contains "2".
  20. Validate the 'from_merge' field contains "1".
  21. Validate the 'merge_date' and 'merge_time' fields contain the date/time of the 'Client Merge'.
  22. Close the report.
Scenario 2: Change MR# - Validate the 'cw_observations_detail_archive' SQL table
Specific Setup:
  • A client must be enrolled in an existing episode and have 'Client Observation' records on file from 30+ days ago that display in the 'cw_observations_detail_archive' table (Client A).
Steps
  1. Access Crystal Reports or other SQL Reporting tools.
  2. Select the CWS namespace.
  3. Create a report using the 'cw_observations_details_archive' SQL table.
  4. Navigate to the existing row for the record for "Client A".
  5. Validate the 'PATID' field contains the ID for "Client A"
  6. Access the 'Change MR# ' form.
  7. Enter the client in the field of 'Client ID'.
  8. Click [Assign MR#].
  9. Validate a confirmation message is displayed stating: "Auto Assign Next ID Number?".
  10. Click [Yes].
  11. Validate the 'New Client ID#' contains the new MR# that will be assigned to "Client A".
  12. Click [Submit].
  13. Access Crystal Reports or other SQL Reporting tools.
  14. Refresh the report using the 'cw_observations_details_archive' SQL table.
  15. Navigate to the existing row for the record for "Client A".
  16. Validate the 'PATID' field contains the updated MR# for "Client A".
  17. Close the report.

Scenario 1: Progress Notes (Group and Individual) - Validate Site Specific Fields
Specific Setup:
  • Must have the 'SS Note Scrolling Free Text 111' and 'SS Note Scrolling Free Text 136' fields added to the 'Progress Notes (Group and Individual)' form. This can be done in 'Site Specific Section Modeling'.
  • A client is enrolled in an existing episode (Client A).
Steps
  1. Access the 'Form and Table Documentation' form.
  2. Select "Form" in the 'Type of Documentation' field.
  3. Select "Individual Form" in the 'Individual or All Forms' field.
  4. Select "Progress Notes->Progress Notes (Group and Individual)' in the 'Form to Be Documented' field.
  5. Click [Process].
  6. Validate the 'Form and Table Documentation' report is displayed for the 'Progress Notes (Group and Individual)' form.
  7. Click [Next Page] until you get to the documentation for the 'SS Note Scrolling Free Text 111' and 'SS Note Scrolling Free Text 136' fields.
  8. Validate the 'SS Note Scrolling Free Text 111' field contains the following:
  9. Table Name: SYSTEM.cw_patient_notes_supp_6
  10. Column Name: ss_note_scroll_text_111
  11. Validate the 'SS Note Scrolling Free Text 136' field contains the following:

Scenario 2: Progress Notes (Group and Individual) Copy - Validate Site Specific Fields
Specific Setup:
  • Must have the 'SS Note Scrolling Free Text 111' and 'SS Note Scrolling Free Text 136' fields added to the 'Progress Notes (Group and Individual) Copy' form. This can be done in 'Site Specific Section Modeling'.
  • A client is enrolled in an existing episode (Client A).
Steps
  1. Access the 'Form and Table Documentation' form.
  2. Select "Form" in the 'Type of Documentation' field.
  3. Select "Individual Form" in the 'Individual or All Forms' field.
  4. Select "Progress Notes->Progress Notes (Group and Individual) Copy' in the 'Form to Be Documented' field.
  5. Click [Process].
  6. Validate the 'Form and Table Documentation' report is displayed for the 'Progress Notes (Group and Individual) Copy' form.
  7. Click [Next Page] until you get to the documentation for the 'SS Note Scrolling Free Text 111' and 'SS Note Scrolling Free Text 136' fields.
  8. Validate the 'SS Note Scrolling Free Text 111' field contains the following:

Chart View - 'Vitals Entry'
Note - These testing guidelines assume the user is skilled in the use of, at a minimum, the following:
  • Vitals Entry
  • Recent Vitals Widget
Scenario 1: Vitals Entry - Validate the 'Chart View'
Specific Setup:
  • The 'Vitals Entry' form must be added to the Chart View.
  • A client is enrolled in an existing episode (Client A).
Steps
  1. Double click on "Client A" in the 'My Clients' widget.
  2. Validate the 'Chart View' is displayed.
  3. Select "Vitals Entry" from the left-hand side.
  4. Click [Add].
  5. Validate the 'Vitals Entry' form opens.
  6. Select "Add" in the 'Update Vital Sign' field.
  7. Enter the desired date in the 'Date' field.
  8. Enter the desired time in the 'Time' field.
  9. Select "No" in the 'Refused Vitals' field.
  10. Populate all remaining vital sign fields.
  11. Click [Submit] and remain in the form.
  12. Navigate to the 'Chart View'.
  13. Click [Refresh].
  14. Validate the vitals added for "Client A" are displayed.
  15. Click [Edit].
  16. Validate the 'Vitals Entry' form opens.
  17. Click [Delete] in the 'Update Vital Sign' field.
  18. Select the vitals record created in the previous steps.
  19. Click [Ok].
  20. Click [Submit] and remain in the form.
  21. Navigate to the 'Chart View'.
  22. Click [Refresh].
  23. Validate the vitals deleted for "Client A" are no longer displayed.
  24. Close the Chart.
Ambulatory and Inpatient Progress Notes - Site Specific Fields
Scenario 1: Ambulatory Progress Notes - Validate Site Specific fields
Specific Setup:
  • Must have a copy of the 'Ambulatory Progress Notes' form (Ambulatory Progress Notes Copy).
  • Must have the 'SS Note Scrolling Free Text 111' and 'SS Note Scrolling Free Text 136' fields added to the 'Ambulatory Progress Notes Copy' form. This can be done in 'Site Specific Section Modeling'.
  • A client is enrolled in an existing outpatient episode (Client A).
  • The 'Progress Notes' widget is accessible from the HomeView.
Steps
  1. Select "Client A" and access the 'Ambulatory Progress Notes Copy' form.
  2. Select "Independent Note" in the 'Note Type' field.
  3. Select the desired value in the 'Note Type' field.
  4. Enter the desired value in the 'Notes Field' field.
  5. Select "Draft" in 'Draft/Final' field.
  6. Enter the desired value in the 'SS Note Scrolling Free Text 111' field.
  7. Enter the desired value in the 'SS Note Scrolling Free Text 136' field.
  8. Click [Submit].
  9. Select "Client A" and access the 'Ambulatory Progress Notes Copy' form.
  10. Select the draft note filed in the previous steps and click [Edit].
  11. Validate all previously filed data is displayed.
  12. Select "Final" in the 'Draft/Final' field.
  13. Click [Submit].
  14. Select "Client A" and access the 'Progress Notes' widget.
  15. Validate the progress note filed in the previous steps is displayed.
Scenario 2: Inpatient Progress Notes - Validate Site Specific fields
Specific Setup:
  • Must have a copy of the 'Inpatient Progress Notes' form (Inpatient Progress Notes Copy).
  • Must have the 'SS Note Scrolling Free Text 111' and 'SS Note Scrolling Free Text 136' fields added to the 'Inpatient Progress Notes Copy' form. This can be done in 'Site Specific Section Modeling'.
  • A client is enrolled in an existing inpatient episode (Client A).
  • The 'Progress Notes' widget is accessible from the HomeView.
Steps
  1. Select "Client A" and access the 'Inpatient Progress Notes Copy' form.
  2. Select "Independent Note" in the 'Progress Note For' field.
  3. Select the desired value in the 'Note Type' field.
  4. Enter the desired value in the 'Notes Field' field.
  5. Select "Draft" in 'Draft/Final' field.
  6. Enter the desired value in the 'SS Note Scrolling Free Text 111' field.
  7. Enter the desired value in the 'SS Note Scrolling Free Text 136' field.
  8. Click [Submit] and stay on the form.
  9. Select the draft note filed in the previous steps and click [Edit].
  10. Validate all previously filed data is displayed.
  11. Select "Final" in the 'Draft/Final' field.
  12. Click [Submit] and close the form.

Update 68 Summary | Details
Event logic in 'Progress Notes' and 'Treatment Plan' forms
Note - These testing guidelines assume the user is skilled in the use of, at a minimum, the following:
  • Progress Notes (Group and Individual)
  • Treatment Plan
  • Treatment Plan Number 1
Scenario 1: Progress Notes (Group and Individual) - File a new service note with document routing enabled
Specific Setup:
  • Document Routing must be enabled on the 'Progress Notes (Group and Individual)' form.
  • A client must be enrolled in an active episode (Client A).
  • Google Chrome Browser settings are set to enable Autofill for passwords and the logged in user's password has been saved.
Steps
  1. Select "Client A" from the 'My Clients' list and access the 'Progress Notes (Group and Individual)' form.
  2. Select any value from the 'Select Episode' field.
  3. Select "New Service" from the 'Progress Notes For' field.
  4. Select any value from the 'Note Type' field.
  5. Set the 'Notes Field' to any value.
  6. Set the 'Date Of Service' field to the current date.
  7. Set the 'Service Charge Code' field to any value.
  8. Select "Final" from the 'Draft/Final' field/
  9. Click [Submit Note].
  10. Validate that the 'Document Routing' dialog is displayed with the progress note data.
  11. Click [Accept].
  12. Validate the 'Password' field autofill's with the user's password that is saved in Google Chrome.
  13. Click [OK].
  14. Validate a 'Progress Notes message' is displayed stating: "Note Filed".
  15. Click [OK].
  16. Click [Discard].
  17. Access the 'Client Ledger' form.
  18. Set the 'Client ID' field to "Client A".
  19. Select "All Episodes" from the 'Claim/Episode/All Episodes' field.
  20. Select "Simple" from the 'Ledger Type' field.
  21. Select "Yes" from the 'Include Zero Charges' field.
  22. Click [Process].
  23. Validate the Client Ledger Report page contains the service created in the previous steps.
  24. Click [Dismiss].
Scenario 2: Progress Notes (Group and Individual) - Validate event logic for signature fields
Specific Setup:
  • The 'Progress Notes (Group and Individual)' form must have the 'SS Signature Pad 1' and 'SS Note Single Response Dictionary 1' fields added via 'Site Specific Section Modeling'.
  • The 'SS Note Single Response Dictionary 1' field must have two dictionary values defined: "Enabled" and "Disabled".
  • The 'SS Note Single Response Dictionary 1' field must have event logic defined with the following:
  • If "Enabled" is selected, it will enable and clear the respective 'SS Signature Pad 1' field.
  • If "Disabled" is selected, it will disable the 'SS Signature Pad 1' field.
  • A client is enrolled in an existing episode (Client A).
  • The 'Progress Notes (Group and Individual)' form is accessible from the Chart View.
Steps
  1. Access the 'Progress Notes (Group and Individual)' form.
  2. Select "Client A" in the 'Select Client' field.
  3. Select "Independent Note" in the 'Progress Note For' field.
  4. Enter the desired value in the 'Notes Field' field.
  5. Select the desired value in the 'Note Type' field.
  6. Click [Sign] in the 'SS Signature Pad 1' field.
  7. Populate the signature and click [OK].
  8. Select "Draft" in the 'Draft/Final' field.
  9. Click [File Note].
  10. Validate a message is displayed stating "Note Filed".
  11. Click [OK] and leave the form opened.
  12. Double click on "Client A" to access the 'Chart View'.
  13. Select the 'Progress Notes (Group and Individual)' form on the left-hand side.
  14. Validate the draft note filed in the previous steps is displayed.
  15. Validate the signature is displayed.
  16. Close the chart.
  17. Navigate back to the opened 'Progress Notes (Group and Individual)' form.
  18. Select the draft note in the 'Select Draft Note To Edit' field.
  19. Validate all previously populated fields are displayed.
  20. Validate the signature is displayed in the 'SS Signature Pad 1' field.
  21. Select "Enable" in the 'SS Single Response Dictionary 1' field.
  22. Validate the signature is cleared from the 'SS Signature Pad 1' field.
  23. Click [File Note].
  24. Validate a message is displayed stating "Note Filed".
  25. Click [OK].
  26. Select the draft note in the 'Select Draft Note To Edit' field.
  27. Validate the signature is no longer displayed in the 'SS Signature Pad 1' field.
  28. Close the form.
  29. Double click on "Client A" to access the 'Chart View'.
  30. Select the 'Progress Notes (Group and Individual)' form on the left-hand side.
  31. Validate the draft note filed in the previous steps is displayed.

  33. Close the chart.
Scenario 3: Treatment Plan - Validate event logic for signature fields
Specific Setup:
  • A client must be enrolled in an existing episode (Client A).
  • The 'SS Treatment Plan Client Dictionary 2' field must have two dictionary values defined: "Enabled" and "Disabled".
  • The 'SS Treatment Plan Client Dictionary 2' field must have event logic defined with the following:
  • If "Enabled" is selected, it will enable and clear the respective 'SS Treatment Plan Client Sign 2' field.
  • If "Disabled" is selected, it will disable the 'SS Treatment Plan Client Sign 2' field.
  • The 'Treatment Plan' form is accessible from the Chart View.
Steps
  1. Select "Client A" and access the 'Treatment Plan' form.
  2. Enter the desired date in the 'Plan Date' field.
  3. Enter the desired value in the 'Plan Name' field.
  4. Select the desired value in the 'Plan Type' field.
  5. Click [Sign] in the 'SS Treatment Plan Client Sign 2' field
  6. Populate the signature and click [OK].
  7. Select "Draft" in the 'Treatment Plan Status' field.
  8. Click [Submit].
  9. Double click on "Client A" to access the 'Chart View'.
  10. Select the 'Treatment Plan' form on the left-hand side.
  11. Validate the draft treatment plan filed in the previous steps is displayed.
  12. Validate the signature is displayed.
  13. Close the chart.
  14. Navigate back to the 'Treatment Plan' form.
  15. Select the treatment plan filed in the previous steps and click [Edit].
  16. Validate all previously populated fields are displayed.
  17. Validate the signature is displayed in the 'SS Treatment Plan Client Sign 2' field.
  18. Select "Enable" in the 'SS Treatment Plan Client Dictionary 2' field.
  19. Validate the signature is cleared from the 'SS Treatment Plan Client Sign 2' field.
  20. Click [Submit].
  21. Double click on "Client A" to access the 'Chart View'.
  22. Select the 'Treatment Plan' form on the left-hand side.
  23. Validate the draft note filed in the previous steps is displayed.

Scenario 4: Treatment Plan - File a Treatment Plan with Document Routing
Specific Setup:
  • Client is enrolled in an existing episode (Client A)
  • The 'Treatment Plan' form must have document routing enabled.
  • Must have the 'My To Do's' widget configured on a view.
Steps
  1. Select "Client A" and access the 'Treatment Plan' form.
  2. Click [Add].
  3. Enter "T" in the 'Plan Date' field.
  4. Validate the current date is displayed in the 'Plan Date' field.
  5. Click [Plan Date T].
  6. Validate the current date is displayed in the 'Plan Date' field.
  7. Select any value in the 'Plan Type' field.
  8. Select any value from 'Problem List'.
  9. Click [New Row - Plan Participants].
  10. Select any value from the 'Role' field in the 'Participation' section.
  11. Select 'Staff ID' and enter "Staff Member A".
  12. Validate that the selected staff member's name displays in the 'Participant Name' field.
  13. Select any value from the 'Plan Author' field.
  14. Select any value from the 'Notification' field.
  15. Add multiple staff members as needed.
  16. Enter any value in the 'Strengths' field.
  17. Enter any value in the 'Weakness' field.
  18. Enter any value in the 'Discharge Planning' field.
  19. Select "Draft" in the 'Draft/Final' field.
  20. Click [Launch Plan].
  21. Select the problem from the 'Tree View'.
  22. Select any value from the 'Status' field.
  23. Click [Add New Goal].
  24. Validate the 'Tree View' displays the new entry.
  25. Enter multiple lines of text in the 'Goal' field.
  26. Select any value from the 'Status' field.
  27. Click [Add New Objective].
  28. Validate the 'Tree View' displays the new entry.
  29. Enter multiple lines of text in the 'Objective' field.
  30. Select any value from the 'Status' field.
  31. Click [Add New Intervention].
  32. Validate the 'Tree View' displays the new entry.
  33. Select any value in the 'Status' field.
  34. Click [Return to Plan].
  35. Select "Final" in the 'Draft/Final' field.
  36. Click [Submit].
  37. Validate the treatment plan data displays as expected in the 'Document Routing' screen.
  38. Click [Sign and Route].
  39. Enter the password and click [Verify].
  40. Select the staff member associated with the logged in user in the 'Route Document To' field and click [Add]
  41. Click [Submit]
  42. Access the 'My To Do's' widget.
  43. Click [My To Do's].
  44. Validate the record for "Client A" and click [Review].
  45. Validate the treatment plan data displays as expected in the 'Document Preview'.
  46. Click [Accept] and [Sign].
  47. Enter the password and click [Verify].
  48. Validate the record is no longer present.
  49. Close the 'To Do's'.
Treatment Plan - 'Enable Automatic Backup'
Note - These testing guidelines assume the user is skilled in the use of, at a minimum, the following:
  • Registry Settings (CWS)
  • Treatment Plan
  • Treatment Plan Number 1
Scenario 1: Treatment Plan - validate toggling of the 'Enable Automatic Backup' registry setting
Specific Setup:
  • The 'Avatar CWS->Treatment Plan->Enable Automatic Backup' registry setting must be set to "Y".
  • A client must be enrolled in an existing episode (Client A).
Steps
  1. Select "Client A" and access the 'Treatment Plan' form.
  2. Enter the desired date in the 'Plan Date' field.
  3. Enter the desired value in the 'Plan Name' field.
  4. Select the desired value in the 'Plan Type' field.
  5. Select "Draft" in the 'Treatment Plan Status' field.
  6. Click [Launch Plan].
  7. Add a new problem and populate all required and desired fields.
  8. Click [Back To Plan Page] and close the form.
  9. Select “Client A” and access the ‘Treatment Plan’ form.
  10. Validate the ‘Load From Backup’ dialog displays with a message stating: "You have an unsubmitted backup of a plan from [the current date] and [the current time]. Would you like to load it instead of creating a new one?"
  11. Click [Yes].
  12. Validate the backed up treatment plan data displays.
  13. Leave the form open.
  14. Access the 'Registry Settings' form.
  15. Enter "Enable Automatic Backup" in the 'Limit Registry Settings To The Following Search Criteria' field.
  16. Click [View Registry Settings].
  17. Enter "N" in the 'Registry Setting Value' field.
  18. Click [Submit] and close the form.
  19. Navigate back to the 'Treatment Plan' form.
  20. Click [Submit].
  21. Select "Client A" and access the 'Treatment Plan' form.
  22. Validate the pre-display contains the draft filed in the previous steps.
  23. Click [Edit].
  24. Validate the previously filed data is displayed.
  25. Close the form.
Scenario 2: Treatment Plan - File a Treatment Plan with Document Routing
Specific Setup:
  • Client is enrolled in an existing episode (Client A)
  • The 'Treatment Plan' form must have document routing enabled.

  8. Select the desired value in the 'Type of Admission' field.
  9. Select the desired staff in the 'Admitting Practitioner' field.
  10. Select the "Demographics" section.
  11. Enter the desired value in the 'Client Middle Name' field.
  12. Enter the desired value in the 'Alias' field.
  13. Enter the desired value in the 'Alias 2' field.
  14. Enter the desired value in the 'Alias 3' field.

  • The 'Attach Selected Service To Notes On Draft' registry setting must be set to "1" or "2" for the 'Progress Notes (Group and Individual)' form.
Steps
  1. Access the 'Client Charge Input' form.
  2. Enter the desired date in the 'Date Of Service' field.
  3. Select "Client A" in the 'Client ID' field.
  4. Select "Service Code A" in the 'Service Code' field.
  5. Click [Submit] and close the form.
  6. Select "Client A" and access the 'Progress Notes (Group and Individual)' form.
  7. Select "Existing Service" in the 'Progress Note For' field.
  8. Select the created service in the 'Note Addresses Which Existing Service/Appointment' field.
  9. Select the desired value in the 'Note Type' field.
  10. Enter the desired value in the 'Notes Field' field.
  11. Select "Draft" in 'Draft/Final' field.

  • OrderConnect - Current Medication Profile
Scenario 1: Clinical Reconciliation - Verify 'New Medication To Add' field will populate the 'Launch OrderConnect' screen.
Specific Setup:
  • Registry Setting 'Avatar CWS->Other Chart Entry->Clinical Reconciliation->->->Enable Clinical Reconciliation' must be set to 'Y'.
  • Registry Setting 'Avatar CWS->Other Chart Entry->Clinical Reconciliation->->->Enable Improved Clinical Reconciliation Workflow' is set to 'Y'.
  • Registry Setting 'Avatar OE-to-OrderConnect Environment' is enabled.
  • "Launch OrderConnect" is set up and enabled.
Steps
  1. Open 'Clinical Reconciliation'
  2. Click the 'Home Medications' tab.
  3. Click [New Row].
  4. Set 'Type to Add' to 'OrderConnect'.
  5. Search for any medication in the 'New Medication to Add' field.
  6. Set the 'Dose' field to a dose for the medication.
  7. Set the 'Dose Unit' to the unit for the medication.
  8. Set the 'Frequency' to any administration frequency.
  9. Place a checkmark in the box to the left of the 'Row ID'
  10. Click [Include].
  11. Click [Review Final List].
  12. Select an 'Episode' from the 'Select Episode' pop up window.
  13. Click [OK].
  14. Review the list of medications on the 'Clinical Reconciliation' - Preview' window.
  15. Click [Reconcile Items].
  16. Click [OK] on the warning 'Saved' pop up window
  17. Click [Close Form].
  18. Open 'Launch OrderConnect' for the same client.
  19. On the episode selection screen, select the client episode the medications were added to in the above steps.
  20. Click [OK].
  21. Click [Launch OrderConnect] to display the 'Current Medication Profile' screen.
  22. Verify the 'Medication Order' screen displays the medication added in 'Clinical Reconciliation'.
  23. Click on the medication order.
  24. Verify the 'Non-OrderConnect Rx' screen displays the medication.
  25. Click [Close Chart]
  26. Close 'Launch OrderConnect' form.
