Client Medication Reconciliation Report

Generate a report that details clients with medication changes filed in the Medication Reconciliation (2011 Edition) form.

Avatar CWS > Other Chart Entry > Client Medication Reconciliation Report

  1. In the Select Client screen, enter the client ID in the Client Name/ID field, click Select Client Name/ID Search. Click OK.
  2. In the Client field, enter the client name, click Process Search to select. Click OK.
  3. In the Episode field, select the client episode.
  4. In the Start Date field, enter the report start date.
  5. In the End Date field, enter the report end date.
  6. Click Process.
    The report displays information filed for the client in the Medication Reconciliation form

If the report does not display the last date and time the dose was taken, please use the Avatar_CWS_Client_Medication_Reconciliation.rpt report.
