Client List/Reminder List

Generate a report that details patient lists, and document reminders that need to be given to client.

This form does not actually generate a patient reminder. It is used solely to record that a reminder was given.

  • The fields in this form are used to filter the display of the Patient Reminder List report.

Client List Report

  1. In the List Type field, select Client List.
    Enter information in the following fields to filter the display of the Client List report.

  2. In the List From Date field, enter the start date.
  3. In the List Through Date field, enter the end date.
  4. In the Gender field, select the client gender.
  5. In the Age Range fields, enter the youngest and oldest ages to include.
  6. In the Service Start Date, Service Start Time fields, enter the start date and time.
  7. In the Service End Date, Service End Time fields, enter the end date and time.
  8. In the Programs field, select the programs to include.
  9. In the Communication Preference field, select the method to include.
  10. In the Problem Code field, enter the problem code, and select.
    Multiple problems can be entered and display for selection in the field below.
    Problems are entered into myAvatar in the Problem List form.
  11. In the Medications field, enter the medication, and select.

  12. In the Test Results field, enter the test result, and select.
    Multiple test results can be entered and display for selection in the field below.​
  13. In the Test Results Abnormals Only field:
    Select Yes to display information for clients with abnormal test results.
    Select No to display all test results.
  14. In the Allergies field, search by specific allergies.
    Allergies populate from the Allergies and Hypersensitivites form.

  15. In the Immunizations field, select the immunizations.
    Immunizations populate from the Client Health Maintenance form.

  16. In the Wellness Items field, select the wellness items to include.
    Wellness items populate from the Client Health Maintenance form.

  17. In the Primary Sort field, select the primary sort for the report.
    If no selection is made in this field, the report will be sorted by client's last name.
  18. In the Secondary Sort field, select the secondary sort category for the report.
    If no selection is made in this field, the report will be sorted by client's last name.
  19. In the Primary Order field, select the primary sort order.
  20. In the Secondary Order field, select the secondary sort order.
  21. Click Display Report.
    The report displays client demographic data, and the following:
  • Client record number
  • Client treatment program
  • If client is an inpatient, the client inpatient location
  • Active medications
  • Active problems
  • Test results
  • Immunizations
  • Wellness information

Reminder List

  1. In the List Type field, select Reminder List.

  2. In the Action field (for client reminders):

  • Select Add to enter a new client reminder.
  • Select Edit to edit an existing client reminder.
  • Select Show Report to view a report on client reminders.
  1. In the Client ID field, select the client (for client reminders).

  2. In the Client Status field, select whether to search for an active or an inactive client (for client reminders).

  3. In the List From Date, and List Through Date fields, enter the start and end dates.

  4. In the Gender field, select the client gender.

  5. In the Age Range field, enter the youngest and oldest ages to include.

  6. In the Service Start Date, and Service Start Time field, enter the service start date and time.

  7. In the Service End Date, and Service End Time field, enter the service end date and time.
  8. In the Programs field, select the programs (for client reminders).

  9. In the Communication Preference field, select the client communication preferences.
    Communication preferences populate from the client's demographic information.

  10. Click Update Client Reminders to launch the Client Reminders screen.

  11. Click Display Report to generate the report (Show Report selected in the Action field).

  12. In the Problem Code field, enter the problem code and select.
    Multiple problem codes can be entered, and display in the field below.

  13. In the Medication Code field, enter the medication code and select.
    Multiple medication codes can be entered, and display in the field below.
    Medications populate from myAvatar OrderConnect, or myAvatar Order Entry.

  14. In the Test Code field, enter the test result code and select.
    Multiple test results can be entered, and display in the field below.

  15. In the Test Result From Date, and Test Result End Date field, select the start and end date for test results to display.
  16. In the Test Results Abnormal Only field, select Yes if only abnormal test results should display.
  17. In the Allergies field, search by specific allergies.
    Allergies populate from the Allergies and Hypersensitivites form.

  18. In the Immunizations field, select the immunizations.
    Immunizations populate from the Client Health Maintenance form.

  19. In the Wellness Items field, select the wellness items to include.
    Wellness items populate from the Client Health Maintenance form.

  20. In the Primary Sort field, select the primary sort category for the report.'

  21. In the Primary Order field, select the sort order.

  1. In the Secondary Sort field, select the secondary sort category for the report.
    In the Secondary Order field, select the sort order.

  2. In the Reminder Type field, search by reminder type.

  • When Add is selected in the Action field:
    Selecting a reminder in this field will filter the display of clients in the Update Client Reminders screen.
    This screen will display clients who do not have an opened ended reminder of the type selected in this field.
  1. In the Reminder Created By field, select the reminders created by specific users.
  2. In the 'Reminders Create Date' fields, enter a date range to search.

  3. In the Reminders Without Complete Date field:

  • Select Yes to include reminders with incomplete dates in the search.
  • Select No to exclude reminders with incomplete dates from the search.
  1. Click Update Client Reminders.
