New Referrals - Cal-PM

You can use the New Referrals form to create a new referral, and send that referral to a provider. If you want to view the created referrals, you can use the CareConnect Inbox form. Please note that:

  • When a Referral is associated to a client, the CCD and any attached documents will be filed in Clinical Document Viewer.
  • Referral attachments are viewable as PDFs.

Prerequisites:

  • Referrals, and the providers who can be sent these referrals, must be defined in the Referral Management Configuration form.
  • The client must be entered in myAvatar.
  1. In the Client field, enter the client name and select.
    Result: Client demographic information populates from Avatar.
  2. In the Reason for Referral field, select the reason this client is being referred.
  3. In the Include Inactive Items field, select whether or not to include inactive items.
  4. In the Start Date and End Date fields, enter the date range for this referral.
  5. In the Purpose field, select the type of referral.
  6. In the Transmit CCD Via field, select:
    • CCD Only
      • In the Receiving Office text field, enter the office that will be receiving the referral.
      • Select whether or not to encrypt the exported information.
      • If you select Yes, indicate if the file should Display Characters Of Key Phrases, and enter an identifying phrase in the Key Phrase For Encryption field.
    • CareConnect
      • In the Receiving Office search field, select the office to receive the referral through CareConnect. This field verifies that the client has a Consent for Access filed and the receiving office is entered into the Referral Management configuration.
      • Select whether to sort the search by Provider Name or Specialty.
      • Include any Additional Notes that apply.
    • Paper
      • In the Receiving Office text field, enter the office that will be receiving the referral.
    • No CCD
      • In the Receiving Office text field, enter the office that will be receiving the referral.
  7. In the Referring Practitioner field, select the practitioner who is referring this client.
  8. Click View Referral Packet to view an abbreviated version of the Referral Detail screen where users may view the CCD and/or generate documents to attach to the referral.
    The type of documents which can be attached is based on the forms selected in the Valid Attachment Types field on the Referral Management Configuration form.
  9. Click Process.
