CSI Assessment-Call Intake - Cal-PM

Enter required assessment information for a client who has been admitted to a Client Services Information System (CSI) treatment program.

  • CSI data is reported to the California Department of Mental Health.

Note: The CSI Assessment (Call Intake) workflow will support scenarios in which a client does not have an Assessment Start Date and the County Client Number (CCN) may be reported as all zeroes. The Call Intake form must be utilized to establish a new provisional client within myAvatar. The client will be assigned a P number instead of a permanent medical record number.


  1. Go to: Avatar PM > Client Management > California Required EDI
  2. Enter the Date of First Contact to Request Services.
  3. Select the Referral Source.
    • The dropdown is defined in PM Dictionary Client File Entity Data Element #73001.
  4. Enter the Assessment Appointment First Offer Date.
    • This date must be greater than or equal to the Date of First Contact to Request Services field.
  5. Enter the Assessment Appointment Second Offer Date and Assessment Appointment Third Offer Date, if applicable.
    • The Assessment Appointment Second Offer Date field will enable once a date is input in the Assessment Appointment First Offer Date field.
    • The Assessment Appointment Third Offer Date field will enable once a date is input in the Assessment Appointment Second Offer Date field.
    • These dates must be greater than the previous dates (second offer date greater than first, third offer date greater than second).
  6. Enter the Assessment Appointment Accepted Date.
    • This field will enable once a date is input in the Assessment Appointment First Offer Date field.
    • If the Assessment Appointment Second Offer Date and Assessment Appointment Third Offer Date fields are empty, then the accepted date must equal the Assessment Appointment First Offer Date.
    • If the Assessment Appointment Third Offer Date field is empty, then the accepted date must equal the Assessment Appointment Second Offer Date.
    • If the Assessment Appointment Third Offer Date field is populated, then the accepted date must be greater than or equal to the Assessment Appointment Third Offer Date.
  7. Select a Closure Reason.
    • The selection is defined in PM Client File Entity Data Element #73010.
  8. Enter the Closed Out Date.
    • This field is utilized when the client starts an assessment or is in contact but does not successfully complete the assessment process.
  9. Select a Referred To option.
    • The selection is defined in PM Client File Entity Data Element #73012.
  10. Select whether or not to Include in CSI Submission?
    • Select whether or not the record is complete and should be included in the monthly submission process.
