CSI Assessment - Cal-PM

Enter required Assessment information for a client who has been admitted to a Client Services Information System (CSI) treatment program.

  • CSI data is reported to the California Department of Mental Health.
  • A client must be admitted into a Pre-Admit or Admission program and have an assigned Medical Record Number. For use with the CSI Assessment form, the selected program must be defined as a Mental Health Program (CSI) in Program Maintenance.
  • A treatment program must be associated with CSI data collection in the Program Maintenance form.
  • A client must have been admitted to a CSI program in the Admission or Admission (Outpatient) form.

  1. Go to: Avatar PM > Client Management > California Required EDI
  2. Enter the Date of First Contact to Request Services.
  3. Select the Referral Source.
    • The dropdown is defined in PM Dictionary Client File Entity Data Element #73001.
  4. Enter the Assessment Appointment First Offer Date.
    • This date must be greater than or equal to the Date of First Contact to Request Services field
  5. Enter the Assessment Appointment Second Offer Date and Assessment Appointment Third Offer Date, if applicable.
    • The second offer date field will enable once a date is input in the Assessment Appointment First Offer Date field.
    • The third offer date field will enable once a date is input in the Assessment Appointment Second Offer Date field.
    • These dates must be greater than the previous date fields (second greater than first, third greater than second)
  6. Enter the Assessment Appointment Accepted Date.
    • This field will enable once a date is input in the Assessment Appointment First Offer Date field.
    • If the Assessment Appointment Second Offer Date and Assessment Appointment Third Offer Date fields are empty, then the Assessment Appointment Accepted Date must equal the Assessment Appointment First Offer Date.
    • If the Assessment Appointment Third Offer Date field is empty, then the accepted date must equal the Assessment Appointment Second Offer Date.
    • If the Assessment Appointment Third Offer Date field is populated, then the accepted date must be greater than or equal to the Assessment Appointment Third Offer Date.
  7. Enter the Assessment Start Date.
    • This field will enable once a date is input in the Assessment Appointment Accepted Date field, and must be greater than or equal to the Assessment Appointment Accepted Date field.
  8. Enter the Assessment End Date.
    • This field will enable once a date is input in the Assessment Start Date field, and must be greater than or equal to the Assessment Start Date field.
  9. Enter the Treatment Appointment First Offer Date.
    • This field will enable once a date is input in the Assessment Start Date field, and must be greater than or equal to the Assessment Start Date field.
  10. Enter the Treatment Appointment Second Offer Date and the Treatment Appointment Third Offer Date, if applicable.
    • The second offer date field will enable once a date is input in the Treatment Appointment First Offer Date field.
    • The third offer date field will enable once a date is input in the Treatment Appointment Second Offer Date field.
    • These dates must be greater than the previous date fields (second greater than first, third greater than second)
  11. Enter the Treatment Appointment Accepted Date.
    • This field will enable once a date is input in the Treatment Appointment First Offer Date field.
    • If the Treatment Appointment Second Offer Date and Treatment Appointment Third Offer Date fields are empty, then the Treatment Appointment Accepted Date must equal the Treatment Appointment First Offer Date.
    • If the Treatment Appointment Third Offer Date field is empty, then the accepted date must equal the Treatment Appointment Second Offer Date.
    • If the Treatment Appointment Third Offer Date field is populated, then the accepted date must be greater than or equal to the Treatment Appointment Third Offer Date.
  12. Enter the Treatment Start Date.
    • This date must be greater than or equal to the Treatment Appointment Accepted Date field.
  13. Select a Closure Reason.
    • The selection is defined in PM Client File Entity Data Element #73010.
    • This field must be blank if a date is entered in the Treatment Start Date field.
  14. Enter the Closed Out Date.
    • This field is utilized when the client starts an assessment or is in contact but does not successfully complete the assessment process.
  15. Select a Referred To option.
    • The selection is defined in PM Client File Entity Data Element #73012.
    • Referred To Other is enabled when Other (Specify) is selected.
  16. Select whether or not to Include in CSI Submission?
    • Select whether or not the record is complete and should be included in the monthly submission process.

Note: Site Specific Modeling is available for the CSI Assessment form. Data submitted through site specific fields will not be part of the CSI submission. The following additional fields are available:
