PI Assessment Appointment

Performance indicator (PI) assessment appointment records entered in Avatar MSO are sent to ProviderConnect.

Details

PI assessment appointments display information filed in ProviderConnect and in the PI Assessment Appointment Form form (Avatar MSO).

PI assessment appointment records:

  • can be entered and edited in either ProviderConnect or in the PI Assessment Appointment Form form (Avatar MSO).

  • filed in ProviderConnect are sent to Avatar MSO, and from Avatar MSO to the Michigan state agency.

Steps

  1. In the Main Menu click Lookup Client.

  • The Search Criteria screen displays.
  1. Enter the client's name, ID, social security number.

  2. Click Search By Criteria.

  • The Search Results screen displays.
  1. Click a client ID number.

  • The ProviderConnect Demographic screen displays for the client and agency.
  1. In the Client Sidebar menu click Appointments.

  • The Appointments screen displays.

Create a PI Assessment Appointment

  1. In the Assessment Appointments section, click Add Assessment Appointment Record.

  • The Assessment Appointment screen displays.
  1. Enter the first assessment request date.

  • Enter dates in the format m/d/yyyy.

  1. Enter the assessment date.

  • Select whether the client refused an appointment scheduled in the 14 calendar period.
  • If Yes, enter the appointment date, and enter associated comments.
  1. Select whether the client refused to reschedule an appointment.

  • If Yes, enter the requested reschedule date, and the actual rescheduled appointment date.
  1. Select whether the client was present for the appointment.

  2. Click Save Record.

Edit a PI assessment appointment

  1. In the Assessment Appointments section, click a record date.

  • The Assessment Appointment screen displays.
  1. Edit selections as appropriate, click Save Record.

Generate the Audit Log Report to view records sent between ProviderConnect and Avatar MSO.
