PI Detox Aftercare Plan

Details

Performance indicator (PI) detox aftercare plan records entered in Avatar MSO are sent to ProviderConnect.

PI detox aftercare appointment records:

• can be entered and edited in either ProviderConnect or in the PI Detox Aftercare Appointment Form form (Avatar MSO).

• filed in ProviderConnect are sent to Avatar MSO, and from Avatar MSO to the Michigan state agency.

1. In the Main Menu click Lookup Client.

• The Search Criteria screen displays.

2. Enter the client's name, ID, social security number.

3. Click Search By Criteria.

• The Search Results screen displays.

4. Click a client ID number.

• The ProviderConnect Demographic screen displays for the client and agency.

5. In the Client Sidebar menu click Appointments.

• The Appointments screen displays.

Create a PI Detox Aftercare Appointment Plan

1. In the Detox Aftercare Plan section, click Add Detox Aftercare Plan.

• The Detox Aftercare Plan screen displays.

2. Enter the detox discharge date in the format m/d/yyyy.

3. Select whether the client chose not to use CA, CMHSP, or PIHP services.

4. Select whether the client requested an appointment outside of the seven day period.

5. Select whether the client refused an appointment date outside of the seven day period.

• If Yes, enter the date of the refused appointment.

6. An appointment was rescheduled in the seven day period.

• If Yes, enter the scheduled appointment date.

7. Select whether the client was present for the appointment.

• If No, enter the missed appointment date.

8. Enter comments associated with the detox aftercare plan.

9. Click Save Record.

Edit a PI Detox Aftercare Appointment Plan

1. In the Detox Aftercare Plan section, click a record date.

2. Edit selections as appropriate, click Save Record.

Generate the Audit Log Report to view records sent between ProviderConnect and Avatar MSO.
